Les cancers représentent le premier facteur de risque de thrombose, justifiant une fréquence importante de co-prescriptions entre anticancéreux oraux et antithrombotiques, associations potentiellement à risque d'interactions médicamenteuses. L'objectif était d'élaborer un outil synthétique permettant de visualiser les interactions entre anticancéreux oraux et antithrombotiques, puis de proposer une première évaluation de son utilité en pratique quotidienne. Toutes les interactions médicamenteuses ont été recherchées dans Drugs.com® et Thériaque®, puis lors d'une interaction avérée sur DDI Predictor® et PubMed. Les mécanismes d'action et les interactions avec les cytochromes ont également été recensés. L'outil a été testé grâce à une analyse rétrospective des bilans médicamenteux des patients ambulatoires sous anticancéreux oraux bénéficiant du suivi pluridisciplinaire ville-hôpital Oncoral aux Hospices Civils de Lyon entre le 1er janvier 2018 et le 31 décembre 2019. Au total, 23% de toutes les combinaisons testées faisaient l'objet d'une mise en garde. La population choisie pour tester l'outil regroupait 228 patients traités par ATCO et ATH. Le top 3 des IM ATCO/ATH de grade majeur/contre-indiqué était Ibrutinib/Apixaban (n=5), Ibrutinib/Enoxaparine (n=3) et Ibrutinib/Acétylsalicylate de Lysine (n=2). Des effets indésirables hématologiques (n=22) pouvant être imputés aux interactions ATCO/ATH ont été retrouvés chez 11/29 patients présentant une IM ATCO/ATH. L'outil développé a permis d'identifier une interaction médicamenteuse ATCO/ATH chez 13% (n=29/228) de la population, dont 38% (n=11/29) ayant développé des effets indésirables hématologiques pouvant être imputés à cette interaction Cet outil issu de l'expérience Oncoral constitue un support incontournable pour détecter les interactions médicamenteuses ATCO/ATH et les gérer en temps réel.
