Family planning is a decision made by an individual or couples about how many children one would like to have, when to start having children, when to stop having children and how long to rest between each pregnancy(MOH,2007). A significant number of useful studies have been conducted which account for aggregate fertility trends or explain individual fertility behaviors with some degree of power. It can be firmly asserted that the weakness of fertility research to date is not one of the poor theoretical formulations but its failure to place fertility within its local socio cultural and economic context. This study is an attempt to examine the possible socio-cultural and economic factors affecting women's role in family planning decision making with specific reference to Addis Ketema Sub-City, Addis Ababa. The findings indicate that socio-cultural and economic factors interact on fertility and women's fertility behaviors is greatly influenced by the socio-cultural values attached to child bearing.

The purpose of this study was to determine the factors that play a role in decision making undertaken when a woman is faced with infertility, and whether or not art therapy can be helpful within this process. Seven individuals were interviewed about their infertility experience and offered the chance to participate in an art making activity. These methods of inquiry were selected to uncover the meaning that women ascribed to their infertility experiences. The results indicated that women's decision making regarding their infertility was impacted by social, medical, cultural and religious factors. In addition, the results were unable to show whether or not art therapy is an effective intervention with this population because the researcher was unable to consistently offer the art making activity at the end of each interview. It was found that the art making portion of the interview was not feasibly supported within the overall interview structure. Themes of revising fertility expectations, the impact of others, acceptance, and setting limits were articulated through the particpants' personal narratives. This research explores these themes and expands upon the results to identify possibilities for future research.

The ability of women to safely and effectively control their fertility is a critical health and human rights issue. Family planning provides a range of health, social, and economic benefits for women and their families, yet more than 200 million women worldwide who wish to delay or limit their childbearing are not using any method of contraception. The global health community has recently seen renewed interest in promoting family planning use worldwide. This increased focus on family planning raises questions about how best to deliver reproductive health services to the women who need them, which requires a clearer understanding of how women make decisions about their fertility and how such decisions are carried out. This dissertation explores the reasons why women's stated fertility intentions do not always align with their fertility behaviors or outcomes. The first paper explores the concept of unmet need for family planning by using both quantitative and qualitative methods to better understand why women who want to prevent a pregnancy choose not to use contraception in Luanda Province, Angola. The second paper considers the social context in which women make decisions about their pregnancies in a post-abortion care context in Zanzibar, Tanzania. The third paper examines how women's experiences in early childhood shape their risk of unintended pregnancy later in life in the United States. These dissertation papers provide three distinct examples of how researchers' and policy makers' perceptions of women's reproductive decision-making may fail to recognize or address crucial determinants of fertility intentions, behaviors, and outcomes. In particular, they highlight the importance of considering women's fertility decisions within the context of their full reproductive lives, and point to the need for further research to better understand how such decisions shift over the course of women's lives. Policies and programs that can accommodate how these decisions are formed and changed over time are necessary for ensuring that all women have the right to make decisions about their reproduction and to enact these decisions in safe and effective ways.
