L’hémorragie du post-partum est l’une des premières causes de mortalité maternelle en France et reste le plus souvent imprévisible. On parle d’hémorragie grave du post-partum en cas de saignement ne cédant pas après la mise en route du sulprostone et nécessitant un recours aux techniques invasives d’hémostase (chirurgie ou embolisation pelvienne). L’objectif de notre étude était de mettre en évidence des facteurs prédictifs de gravité ou de guérison, permettant une prise en charge adaptée la moins invasive possible. Notre étude rétrospective a inclus 310 patientes ayant accouché sur place ou ayant été transférées pour hémorragie grave du post-partum à l’hôpital de Lariboisière entre avril 2007 et avril 2009. Soixante-dix pour cent des patientes ont eu une prise en charge médicale seule. Les principaux facteurs significatifs retrouvés entre les patientes ayant eu une prise en charge médicale seule et celles ayant eu une prise en charge invasive sont, au niveau clinique la fréquence cardiaque (88 versus 100 battements par minute, p=4.10-14), l’importance du saignement (p=4.10-77) et la tonicité du globe utérin (p=4.10-18). Et au niveau biologique, les critères de gravité relevés sont le taux d’hémoglobine (9 versus 8.2 g/dL, p=1.5.10-6) et les facteurs de coagulation, avec surtout le taux de fibrinogène (3 versus 2 g/L, p=2.10-12). Les facteurs de guérison identifiés correspondent aux facteurs de gravité. En conclusion, en dehors de l’examen clinique, ce sont surtout les facteurs d’hémostase qui sont pertinents dans les cas d’hémorragie grave du post-partum.
