Almost two decades ago, Drs. Meyer Friedman and Ray Rosenman de veloped the concept of the Type A coronary-prone behavior pattern and pioneered research in the area. Since then, much effort has been devoted to investigating both medical and psychosocial implications of this phenomenon by an impressive array of biomedical and behavioral scientists. On the basis of the National Heart, Lung, and Blood Institute's (NHLBI) recent Congressional mandate concerning disease prevention and control, the Division of Heart and Vascular Diseases undertook an intensive review of the existing literature in this area. The review underscored that the very nature of the concept of coronary-prone behavior requires examination by researchers from a variety of disciplines. Publication of findings in both the medical and behavioral literature, however, has created difficulties in gaining a truly com prehensive understanding of the total effort in this area. It became obvious that there was no coherent integration of information regarding the strength of the association between behaviors and disease processes (or outcomes), how be havioral factors associated with coronary heart disease were measured, the possible physiological mechanisms mediating the relationship between be havior and disease, whether intervention could be effective, and what forms of intervention appeared most promising. In short, a clear need existed to or ganize this information in a more coherent fashion so that it could be subjected to critical review by members of both the medical and behavioral scientific communities.

This book is a cumulation of a research program that began in the sum mer of 1978, when I was a doctoral student at the University of Missouri. What started as a graduate student' s curiosity about individual differ ences in need for personal control led to a personality scale, a few pub lications, some additional questions, and additional research. For reasons I no longer recall, I named this personality trait desire for control. One study led to another, and questions by students and colleagues often spurred me to apply desire for control to new areas and new questions. At the same time, researchers around the globe began using the scale and sending me reprints of articles and copies of papers describing work they had done on desire for contro!. In the past decade or so, I have talked or corresponded with dozens of students who have used the scale in their doctoral dissertation and master's thesis research. I have heard of or seen translations of the Desirability of Control Scale into German, Polish, Japanese, and French. There is also a children's version of the scale. We seek to throw down the gauntlet with this handbook, challenging the he gemony of the "behavioral medicine" approach to the psychological study and treatment of the physically ill. This volume is not another in that growing surfeit oftexts that pledge allegiance to the doctrinaire purity of behavioristic thinking, or conceptualize their subject in accord with the sterility of medical models. Diseases are not our focus, nor is the narrow band of behavioral assessment and therapy methodologies. Rather, we have sought to redefine this amorphous, yet burgeoning field so as to place it squarely within the province of a broadly-based psychology-specifically, the emerging, substantive discipline of health psy chology and the well-established professionalism and diverse technologies of clinical psychology. The handbook's title-Clinical Health Psychology-reflects this reorientation explicitly, and Chapter 1 addresses its themes and provides its justifications more fully. In the process of developing a relevant and comprehensive health assess ment tool, the editors were struck by the failure of clinical psychologists to avail themselves of the rich vein of materials that comprise the psychosocial world of the physically ill. Perhaps more dismaying was the observation that this field was being mined-less than optimally-by physicians and nonclinical psychologists.
