A partir d'une observation personnelle d'endocardite infectieuse mitrale obstructive traitée chirurgicalement, une revue exhaustive des autres cas de la littérature est présentée et commentée, afin de dégager le profil de cette forme clinique particulière. Elle se manifeste cliniquement dans un contexte septicémique par l'apparition de signes d'insuffisance cardiaque gauche rebelles aux thérapeutiques habituelles contrastant avec, à l'auscultation, un petit souffle d'insuffisance mitrale et, parfois, un roulement diastolique. Elle est confirmée par l'échocardiographie qui visualise une masse s'enclavant dans l'orifice mitral. L'évolution est spontanément grave avec un risque permanent et imprévisible de mort subite. Le traitement est chirurgical, exérèse et remplacement valvulaire en semi-urgence, si possible après contrôle du processus infectieux par antibiothérapie.
