L’objectif de notre étude était principalement de déterminer si la prise de poids supérieure aux recommandations Canadiennes chez la femme obèse (11,5 kg) influait sur le taux de césarienne, et secondairement sur les complications obstétricales à savoir le taux d’extractions instrumentales, de déclenchements du travail ou d’hémorragies de la délivrance. Notre étude était une étude de cohorte à visée étiologique comparant un groupe exposé et un groupe non-exposé. Elle concernait toutes les patientes atteintes d’obésité (simple et morbide), ayant eu leur première consultation de grossesse au CHU de Grenoble entre le 1er Janvier 2006 et le 31 Décembre 2007.Le critère de jugement principal était l’accouchement par césarienne. Les critères secondaires étaient le déclenchement du travail, le type d’anesthésie, les extractions instrumentales, la macrosomie ou l’hypotrophie, l’hémorragie de la délivrance et ses thérapeutiques. Ainsi que la durée du travail, la difficulté ou la dystocie des épaules, l’état périnéal et les anomalies de la délivrance. Nos résultats ont montré significativement plus de primipares dans le groupe de femmes ayant pris plus de 11,5 kg ainsi qu’un âge moyen inférieur dans ce groupe. Nous avons retrouvé un taux de macrosomie significativement plus élevé dans le groupe de femmes ayant pris plus de 11,5 kg. L’augmentation des complications obstétricales et néonatales par la prise de poids excessive pendant la grossesse chez la femme obèse rend primordial une meilleure prise en charge de ces femmes avant, pendant et après la grossesse.
