Introduction : La médicalisation systématique des transferts secondaires pour rapprochement de plateau technique des patients avec une lésion ischémique cérébrale aiguë entraîne un coût financier et humain conséquent. A ce jour, il n'existe aucune recommandation pour cette pratique. Cette décision est le fruit d'une concertation entre le médecin régulateur et le neurologue de garde. Le but de cette étude est d'évaluer la pertinence de la médicalisation de ces transferts et de mettre en évidence les facteurs favorisants de complications graves nécessitant une surveillance médicale pendant le transport et ainsi mettre en place un score d'aide à la décision de médicalisation. Matériel et méthode : Cette étude est descriptive rétrospective monocentrique, observationnelle, concerne les transferts secondaires entre les différents CH de proximité de la région Franche-Comté et le CHU de Besançon, sur une période allant du 01/01/2019 au 31/12/2022 inclus. Le critère de jugement principal est la survenue d'une complication dite grave, nécessitant l'action d'un médecin pendant le transport. Les objectifs secondaires incluent la comparaison des paramètres cliniques et de transfert entre les patients ayant présenté une complication grave de ceux n'en ayant pas présenté. Ces résultats ont été obtenus grâce à des analyses univariées et multivariées. Ainsi, en comparant les données de l'étude et celle de la littérature, nous proposons un score prédictif de complication grave. Résultats : Sur l'échantillon de 370 patients, 14% ont présenté une complication dite grave. En analyse univariée comme multivariée, l'état clinique initial avec le score NIHSS, l'état de conscience avec le score de Glasgow, ainsi que la localisation de la lésion cérébrale représentent un facteur de risque de complication. En revanche, les antécédents, les traitements antiagrégants, la réalisation d'une thrombolyse intraveineuse et les paramètres de transfert ne sont pas significatifs. Conclusion : une médicalisation systématique de ces transferts secondaires n'est pas nécessaire mais l'identification des patients à risque est essentielle. La gravité de l'état neurologique initial, souvent corrélée à la localisation de la lésion cérébrale, sont des événements devant influencer la médicalisation du transport. Nous proposons au terme de cette étude un score prédictif de complications graves allant de 0 à 5 points. La décision de médicalisation à partir d'un point, permettrait une réduction de moitié des transferts secondaires pour AVC ischémique en Franche Comté.
