This report highlights key trends and variations in the delivery of healthcare across the four nations of the UK. Life expectancy varies significantly across the UK - from 75.9 in Scotland to 78.6 in England for men, and from 80.4 in Scotland to 82.6 in England for women. Spending on health services in the UK more than doubled in cash terms in the last decade. In 2010-11, despite devoting a higher proportion of total public spending to health, England spent the least on health per person. NHS staff has also increased over the last decade. Scotland has the most GPs per person (80 per 100,000 people in 2009 compared with 70 in England and 65 per 100,000 in both Wales and Northern Ireland). Scotland also has the most medical hospital staff and nursing, midwifery and health visiting staff per person. Comparable data on the efficiency and quality of healthcare are patchy. In 2008-09, average hospital lengths of stay varied from 4.3 days in England to 6.3 days in Wales. Hospital waiting times have fallen in all four nations in recent years, although there are notable variations in how long patients wait for common procedures. In 2009-10 waiting times tended to be lower in England and Wales. There have been significant improvements in levels of healthcare associated infections with rates of MRSA infection dropping by a third or more in all nations. The NAO considers that there would be value in the health departments in the four nations carrying out further work to investigate the variations in performance and identify how they can learn from each other

The UK Government is undertaking a major cross-government programme of analysis prior to the referendum on Scottish independence in 2014. The aim is to provide a comprehensive and detailed analysis of Scotland's place in the UK. This paper, the first of a series to be published in 2013 and 2014, examines the UK's constitutional set-up and the legal implications of independence. The UK Government is convinced that the current devolution offers the best for Scotland: the Scottish Parliament and Government are empowered to take decisions on a range of domestic policy areas - such as health, education, policing - while Scotland continues to benefit from decisions made for the UK as a whole - defence and security, foreign representation, economic affairs. Independence is very different to devolution. Based on independent expert opinion (published as Annex A), the paper concludes that if there were to be a vote in favour of leaving the UK, Scotland would become an entirely new state whilst the remainder of the UK would continue as before, retaining the rights and obligations of the UK as it currently stands. Any separation would have to be negotiated between both governments. Legal and practical implications of independence, both at home and abroad, are addressed. An independent Scotland would have to apply to and/or negotiate to become a member of whichever international organisations it wished to join, including the EU and NATO. Scotland would also have to work through its positions on thousands of international treaties to which the UK is currently party.

This open access book modifies and revitalizes the concept of the ‘developmental state’ to understand the politics of emerging economy through nuanced analysis on the roles of human agency in the context of structural transformation. In other words, there is a revived interest in the ‘developmental state’ concept. The nature of the ‘emerging state’ is characterized by its attitude toward economic development and industrialization. The UK has the most centralised system of government amongst major economies. This results in poorer services, lower economic growth and higher taxes. We have also developed an approach to devolution that is incoherent and unstable. This short book proposes an entirely new set of constitutional arrangements. It proposes that the UK should develop a federal structure of government with only a small number of functions such as defence and border control being determined at the UK level. All other functions would be the ultimate responsibility of individual nations within the UK, though Wales, Northern Ireland and England could combine together if they wished. The author also proposes further radical decentralisation of government. Local government should become responsible for a much wider range of functions and raise the revenue to finance them. In areas such as health and education, the government role would be diminished further as parents, families and civil society institutions are provided with finance to directly procure their own services. Overall, this is a radical plan to completely change the nature of government in the UK.It would return power to the people and reverse the long trend of centralisation that has happened since World War I.
