Chronic homelessness is a highly complex social problem of national importance. The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.

Homelessness is an acknowledged problem in many places, though its causes are myriad and may vary based on the characteristics of respective communities. This report investigates heterogeneity in community-level rates of sheltered and unsheltered homelessness, separately and combined, and provides insight into underlying community-level factors associated with homelessness across the United States. This study (1) identifies and describes market variables associated with sheltered and unsheltered homelessness, (2) constructs and evaluates empirical models of community-level homelessness, and (3) analyzes relationships within subgroup populations of local markets. Findings provide insights into predicting homelessness across different community types and market factors to consider as policy interventions are developed. The study finds that housing factors, such as rental costs, crowding, and evictions, are most consistently associated with higher rates of community-level homelessness. This demonstrates that housing market dynamics and the availability of affordable housing are closely tied to homelessness at the Continuum of Care (CoC) level even when controlling for a range of economic, demographic, safety net, and climate factors.

This book examines the multiple factors which contribute to family homelessness, and uses quantitative and qualitative analyses to identify those factors which represent the major supports and barriers to homeless exit and housing stability. Results show that while family homelessness is not caused by a single issue (but a combination of issues including alcohol/substance abuse, untreated mental illness, domestic violence, family configuration, lack of community or social supports, and/or lack of affordable housing options) community and social supports provide the single most significant impact on the ability of families to exit homelessness and maintain stable housing. Quantitative data suggests that investing in community/social supports for homeless families could facilitate their exit from homelessness and increase housing stability for families at risk, while reducing those supports might increase homelessness. Qualitative data indicates that supportive relationships are instrumental to understanding that while homelessness is a terrible experience, those who experience homelessness are not terrible people. Furthermore, recognizing that a loss of housing does not necessarily mean the loss of one's symbolic representation of 'home' could assist families in viewing homelessness as a transformational learning experience rather than a traumatic failure.

The objective of this research was to study the influence of individual- and community-level conditions on the housing needs among low-income persons living with HIV/AIDS (PLWHA). Secondary data analysis was conducted on HIV/AIDS housing survey data collected in 2006 in a four-county metropolitan area. The study sample consisted of 384 low-income PLWHA living in 78 ZIP Code areas. Community-level data were compiled from 2000 Decennial Census, U.S. Department of Housing and Urban Development, U.S. Department of Agriculture, and other affordable housing databases. Using hierarchical linear modeling, two housing need outcome variables (a) need for housing assistance and (b) housing stability were studied. The first research question concerned the influence of individual conditions (socio-demographics, housing situations, and social histories) on housing need. In predicting need for housing assistance, none of the socio-demographics of age, gender, race, or ethnicity was a significant predictor of housing need. Of the housing situation predictors (housing burden, household composition, potential impact of rent increase, housing subsidy), only housing burden was a statistically significant predictor. Lastly, considering social history (work status, history of homelessness, mental illness and substance use history), a history of homelessness and substance use history were significant predictors. Only a history of homelessness statistically significantly predicted housing stability. The second research question concerned the influence of the community conditions of distress, degree of rurality, and social infrastructure on housing need. All the community predictors, except number of affordable housing units (a measure of social infrastructure), were statistically significant predictors of need for housing assistance. None of the community variables was a statistically significant predictor of housing stability. The findings suggest that community conditions are associated with the need for housing assistance. The third research question concerned interactions between the individual- and community-level conditions. The relationship between history of homelessness and need for housing assistance was stronger in areas where there were more affordable housing. The findings of the study support the conclusion that both individual and community conditions are associated with housing need among PLWHA. The report concludes with a discussion of these results and offers implications for social work practice, policy, and research.
