Les troubles de la communication des personnes traumatisées crâniennes constituent un handicap à leur réinsertion familiale, sociale et professionnelle. Les troubles de la communication verbale sont désormais bien définis mais peu de recherches sur d'éventuels troubles de la communication non verbale ont été menées. L'objectif de ce mémoire a donc été d'étudier la communication et plus précisément la communication non verbale d'adultes traumatisés crâniens graves ayant des troubles des fonctions exécutives. Ainsi ont été examinées auprès de trois patients les hypothèses suivantes : certains présentent un déficit sélectif ou global de la communication non verbale et leurs interlocuteurs s'ajustent spontanément face à ces troubles. Le protocole expérimental utilisé correspond à celui de Sainson (2004) et a été complété par l'élaboration d'une grille d'évaluation des troubles de la communication non verbale. Il comprend : une évaluation neuropsychologique, une évaluation orthophonique de la compréhension élaborée, du discours procédural et de la communication verbale, ainsi qu'une analyse conversationnelle. Les résultats montrent que les patients présentent une altération des niveaux énonciatifs et pragma-interactionnels de la communication non verbale. Cette altération se caractérise par une atteinte sélective ou globale de la posture, de l'intonation, des expressions faciales, du regard et des gestes coverbaux. Ces troubles seraient en lien avec les déficits exécutifs que présentent ces sujets. De plus, une adaptation est observée de la part de leurs interlocutrices qui ajustent communication non verbale, par réciprocité ou par compensation, aux troubles de ces patients.

Speakers tend to compose their utterances in such a way that the message they want to get across is hardly ever fully encoded by the meanings of the words and the grammar they use. Instead speakers rely on hearers adding conceptual and emotive content while interpreting the contextually appropriate meanings and intentions behind utterances. This insight, which is of course particularly relevant in all kinds of indirect, figurative or humorous talk, lies at the heart of the linguistic discipline of pragmatics. If pragmatics is the study of meaning-in-context, then cognitive pragmatics can be broadly defined as encompassing the study of the cognitive principles and processes involved in the construal of meaning-in-context. While it would seem only natural that pragmatics as such should have addressed such cognitive issues anyway, it has mainly been due to the historical rooting of this discipline in the philosophy of language that psychological aspects have not been in the pragmatic limelight to date. Les traumatismes crâniens sont devenus la première cause de handicap sévère avant 45 ans en France, en raison notamment des accidents de la route. Nombre de personnes traumatisées crâniennes connaissent ainsi de lourdes séquelles communicationnelles retentissant sur leur vie familiale et socio-professionnelle. Plusieurs recherches ont été menées sur les troubles de la communication verbale ; mais paradoxalement peu se sont intéressées à la communication non verbale et ses troubles. Les seules études menées sur ce concept ont été réalisées sur un nombre restreint de sujets. C’est pourquoi ce mémoire tend à rendre compte des troubles de la communication verbale et a fortiori non verbale d’un nombre plus important de sujets. Les dix-sept personnes suivies dans cette étude présentent un traumatisme crânien grave et un syndrome dysexécutif. L’objectif de cette recherche était de répondre à quatre hypothèses : la personne traumatisée crânienne grave présente des troubles de la communication verbale et non verbale. Ces troubles peuvent être sélectifs ou globaux. L’interlocuteur ajuste spontanément sa communication à la personne traumatisée crânienne. Il existe une bonne fiabilité inter-juges dans les cotations effectuées. Le protocole d’étude utilisé comprend une évaluation neuropsychologique des fonctions exécutives, une évaluation orthophonique de la compréhension élaborée, du discours procédural et de la conversation ainsi qu’une analyse linguistique d’interactions libres (G.A.L.I., Sainson, à paraître). Les résultats révèlent que les dix-sept sujets de notre étude présentent des troubles, sélectifs ou globaux, de la communication verbale et non verbale. Quant aux dix-sept interlocuteurs, tous s’ajustent, spontanément et inconsciemment, aux troubles communicationnels des sujets avec lesquels ils conversent.
