Pour lutter contre la désertification médicale, le Pacte Territoire Santé de 2012 propose plusieurs solutions, l'une d'entre elle est le développement de la télémédecine. L'Arrêté du 1er Aout 2018 ouvre le remboursement de la téléconsultation en médecine générale si elle a lieu dans le respect du parcours de soin. Ce remboursement sera étendu par la suite par le Décret du 9 Mars 2020 pour les patients suspects de Covid 19. Cette thèse a pour objectif d'analyser comment la téléconsultation modifie la relation entre le médecin et le patient. Nous avons réalisé une étude qualitative en interrogeant individuellement 12 patients entre Janvier et Juillet 2020. Les patients rapportent que la téléconsultation altère la relation avec leur médecin, celui-ci n'ayant pas la même attitude qu'au cabinet. Ils estiment les téléconsultations plus courtes et qu'elles mériteraient une adaptation tarifaire. L'outil informatique est évoqué comme un frein pour les non-initiés, et que les téléconsultations sont altérées par des problèmes d'informatique ou d'internet. Les patients dressent un inventaire des difficultés que les médecins peuvent rencontrer au cours de la téléconsultation. Le médecin redevient alors novice dans la conduite de ces téléconsultations. Ils expriment une grande satisfaction quant aux avantages de la téléconsultation en termes d'organisation, de gain de temps, de confort et de praticité. La possibilité de téléconsulter son médecin traitant quelque soit la distance est rassurante. Cette étude met en évidence la nécessité de rédaction de recommandations de bonnes pratiques et de formation pour les étudiants et les médecins afin de leur permettre d'aborder la téléconsultation de manière optimale.

Introduction : La téléconsultation (TC) est un mode d’exercice en pleine expansion en soin premiers. Elle est définie comme étant : « une consultation à distance entre un professionnel médical et son patient via l’utilisation des nouvelles technologies ». De rares études ont suggéré que la TC avait un impact sur la communication et la relation médecin malade mais aucune revue de la littérature n’avait exploré ce sujet depuis 2001. Cette revue de la littérature étudie l’impact de la TC sur la relation médecin malade et la communication. Méthodologie : Cette revue générale de la littérature intègre les publications issues des principales bases de données scientifiques et médicales. Ont été incluses uniquement les études s’intéressant à la TC pour réaliser une analyse descriptive et thématique des résultats Résultats : Au total, 18 études ont été sélectionnées sur 506 articles. Elles montraient pour la majorité une influence de la TC sur trois axes : les communications verbale et non verbale, altérées avec moins d’échanges mais aussi une difficulté à lire le langage corporel du patient, la construction de la relation médecin malade, aussi impactée du fait de la distance crée par la TC. Discussion : Cette revue de la littérature suggère que la TC influence la communication et la relation médecin malade. D’autres travaux sont à prévoir pour déterminer le type d’impact tant sur un plan qualitatif que quantitatif. Les résultats invitent les médecins généralistes à adapter leurs communications lors des TC.

Introduction : En médecine générale, l'incertitude est fréquente et la science évolue vite. Ainsi, les médecins ont besoin de s'informer. Internet est une source d'information de plus en plus utilisée, aussi bien par les médecins que par les patients. Les conséquences des recherches des patients ont été étudiées à plusieurs reprises. Mais celles des médecins interrogent également. L'objectif de cette étude est de mettre en évidence le rôle sur la relation médecin-patient des recherches internet en consultation. Méthodologie : La méthodologie choisie était une approche qualitative par théorisation ancrée. Des entretiens collectifs en visioconférence ont été réalisés. Résultats : Une influence ambivalente des recherches internet sur la relation médecin-patient a été mise en évidence. Celle-ci tend principalement vers une répercussion positive, permettant au médecin d'accéder à des informations sécurisantes et d'entrer dans une relation plus équilibrée avec son patient. Une certaine vigilance doit tout de même persister, afin de ne pas rompre la relation et se laisser absorber par son écran. Discussion : Selon la littérature, internet peut guider la relation médecin-patient vers une nouvelle ère, avec de nouveaux rôles donnés au médecin. Le médecin devenant guide et formateur du web. Ceci pourrait se développer au sein des maisons de santé pluridisciplinaires.

The fifth international Conference in Medical Image Computing and Computer Assisted Intervention (MICCAI 2002) was held in Tokyo from September 25th to 28th, 2002. This was the first time that the conference was held in Asia since its foundation in 1998. The objective of the conference is to offer clinicians and scientists the opportunity to collaboratively create and explore the new medical field. Specifically, MICCAI offers a forum for the discussion of the state of art in computer-assisted interventions, medical robotics, and image processing among experts from multi-disciplinary professions, including but not limited to clinical doctors, computer scientists, and mechanical and biomedical engineers. The expectations of society are very high; the advancement of medicine will depend on computer and device technology in coming decades, as they did in the last decades. We received 321 manuscripts, of which 41 were chosen for oral presentation and 143 for poster presentation. Each paper has been included in these proceedings in eight-page full paper format, without any differentiation between oral and poster papers. Adherence to this full paper format, along with the increased number of manuscripts, surpassing all our expectations, has led us to issue two proceedings volumes for the first time in MICCAI’s history. Keeping to a single volume by assigning fewer pages to each paper was certainly an option for us considering our budget constraints. However, we decided to increase the volume to offer authors maximum opportunity to argue the state of art in their work and to initiate constructive discussions among the MICCAI audience.

This book examines deformative transparency and its different manifestations in political communication, propaganda and public health. The objective is to present the theoretical foundations of deformative transparency, as grotesque and esperpentic transparency, and illustrate the validity of such approach to understand the strategic and ethical implications of the proactive disclosure of the "shocking", "ugly" or "outside the norm". Four areas are discussed: political communication with particular focus on populist politicians as the deceased Venezuelan president Hugo Chávez, the campaign and presidency of Donald Trump, and the tenure in office of the mayor of Toronto, Rob Ford; propaganda strategies of Islamist terrorist organizations such as the Islamic State’s escalation of the visually horrific; and public health campaigns that use "disturbing images" to promote public awareness and eventually influence behavioural change. This study on the transparently grotesque is part of a research program about the economy of emotions in public communication.
