What are the consequences, if any, of repeated brief seizures that are the defining feature of epilepsy? A firm answer to this question has been surprisingly elusive for a variety of reasons. Indeed, one child out of 20 experiences one or more seizures before the age of 5, and one in a hundred develops epilepsy as a chronic disorder. Approximately half of these children with epilepsy display academic difficulties and/or behavioral disorders. Moreoever, it is now believed that a sizable proportion of children with learning disability suffer from undiagnosed epilepsy. Childhood epilepsy may cause cognitive disorders and the intellectual quotient is indeed not normally distributed in epileptic children, a fair proportion of whom show an IQ in the deficient range. Some epileptic syndromes happen during vulnerability periods of brain maturation and interfere with the development of specific cognitive functions. This is the case for the Landau–Kleffner syndrome, which generally appears during speech development and affects language. Similarly, West syndrome – or infantile spasms – is an epileptogenic encephalopathy appearing during the first years of life and induces a major delay in social and oculo-motor development. Specific impairments can also be identified in partial childhood epilepsies in relation with seizure focus localization. For instance, left temporal and frontal epilepsies are frequently associated with verbal impairments. Moreover, episodic memory disorders have been described in children suffering from temporal lobe epilepsy whereas executive deficits (planning, self-control, problem solving) have been reported in frontal lobe epilepsy. In most cases, including its mildest forms, childhood epilepsy induces attention deficits, which may affect academic achievement. These observations militate in favor of individual neuropsychological assessments as well as early interventions in order to provide the child with an optimal individualized treatment program.

To date, little research has been conducted to the effects of epilepsy on cognitive and language issues. This study is a response to this knowledge gap. A careful matched-subject, mixed-factors description, designed to identify cognitive decline during the process and treatment of this illness. The research is focused on description and exploration, of linguistic considerations in Rolandic epilepsy. Rolandic epilepsy, is also known as Benign focal epilepsy with centrotemporal spikes (BECS). The intention is to shed light from a neurolinguistic perspective, on reasons that might underline cognitive and behavioral complications, in children with epilepsy. This study has been approached corroborating ‘structural connectivity’, which defines language network in the brain. The rational derives from research of neurological data, from prestigious neurological and scientific international journals, on the topic that illustrates our case. Conclusions are based upon careful evaluation on how epilepsy affects cognitive skills, and language issues upon observations drawn on results. Deficits in language patterns have been associated to different brain areas, in patients who suffer from different pathologies. Degenerative illnesses have opened a milestone in the study on reasons leading to cognitive decline, in children affected from “Benign childhood epilepsy”. Medical history has been well documented to current day. Lack of consistent information internationally recognized on cognitive issues toughen consensus on best medical procedure to follow, in best interest of the child. Our research confronted this shortage of information, in the light of the new literature coming through. Overall, the results indicate that when working with language skills in people with epilepsy or with degenerative diseases, clinicians should account for the presence of anti-epileptic medication or amalgamation of medications as possible cause of cognitive decline.

A key construct at the foundation of cognitive and clinical developmental neuropsychology is the notion that cognitive functions are localized to specific cortical regions in the brain. Consistent with this, relatively stable cognitive and behavioral profiles have been described for adults diagnosed with the two most common focal-onset seizure disorders. Temporal lobe epilepsy (TLE) is primarily associated with impairments in memory functioning (Bell & Giovagnoli, 2007) and frontal lobe epilepsy (FLE) with impairments in executive and motor functioning (Patrikelis et al., 2009). However, the immature brain may be particularly vulnerable to the adverse effects of recurrent seizure activity, showing more widespread effects on cognitive processes and brain organization compared to adults (Korman et al., 2013). Despite these observations, few studies have directly compared performance between children with different epilepsy syndromes utilizing broad assessments spanning multiple domains (Williams, Griebel & Dykman, 1998). Consequently, the aim of this study was to evaluate the degree and selectivity of patterns of cognitive and psychosocial dysfunction in children with TLE and FLE. Participants included 51 children between the ages of 6 and 16 years with intractable epilepsy who were consecutively seen for a neuropsychological evaluation through the Dell Children's Medical Center Comprehensive Epilepsy Program. During this assessment, participants were administered a battery including measures of memory, executive, motor and intellectual functioning. In addition, parents completed questionnaires regarding their child's behavioral and psychosocial functioning. Contrary to the selective patterns of deficits typically described in adults, both the TLE and FLE groups demonstrated significant impairments relative to normative values on each of the domains assessed. Moreover, no significant differences were found between the two patient groups on any of the measures, with the exception of a task of visual memory. These findings suggest that individuals with childhood-onset epilepsy exhibit fairly broad patterns of cognitive compromise that do not differ significantly with frontal lobe versus temporal lobe seizure localization. Furthermore, the range of deficits observed would not normally be expected with analogous seizure disorders acquired in adulthood. These results provide important insights into the organization of cognitive and behavioral functions following early neurological insults associated with epilepsy.
