The United States faces an increasingly turbulent world. The risk of violent conflict and other threats to international order presents a vexing dilemma: should the United States remain the principal guarantor of global peace and security with all its considerable commitments and potential pitfalls––not least new and costly military entanglements––that over time diminish its capacity and commitment to play this vital role or, alternatively, should it pull back from the world in the interests of conserving U.S. power, but at the possible cost of even greater threats emerging in the future? Picking up where Minorities at Risk left off, Peoples Versus States offers an expanded and updated perspective on ethnic and nationalist conflict throughout the world, as well as efforts to manage it. Ted Gurr surveys the behavior of 275 politically active ethnic groups during the 1990s and pinpoints the factors that encourage the assertion of ethnic identities. Whereas his highly acclaimed 1993 book presented a disturbing picture of spreading ethnic violence, this volume documents a pronounced decline since the early 1990s--a decline attributable, in part at least, to many states abandoning strategies of assimilation and control in favor of policies of pluralism and accommodation. Nonetheless, Gurr identifies some ninety groups as being at significant risk of conflict and repression in the early 21st century. And he cautions that the emerging global regime of principles and strategies governing relations between communal groups and states is far from perfect or universally effective.

Violent conflicts today are complex and increasingly protracted, involving more nonstate groups and regional and international actors. It is estimated that by 2030—the horizon set by the international community for achieving the Sustainable Development Goals—more than half of the world’s poor will be living in countries affected by high levels of violence. Information and communication technology, population movements, and climate change are also creating shared risks that must be managed at both national and international levels. Pathways for Peace is a joint United Nations†“World Bank Group study that originates from the conviction that the international community’s attention must urgently be refocused on prevention. A scaled-up system for preventive action would save between US$5 billion and US$70 billion per year, which could be reinvested in reducing poverty and improving the well-being of populations. The study aims to improve the way in which domestic development processes interact with security, diplomacy, mediation, and other efforts to prevent conflicts from becoming violent. It stresses the importance of grievances related to exclusion—from access to power, natural resources, security and justice, for example—that are at the root of many violent conflicts today. Based on a review of cases in which prevention has been successful, the study makes recommendations for countries facing emerging risks of violent conflict as well as for the international community. Development policies and programs must be a core part of preventive efforts; when risks are high or building up, inclusive solutions through dialogue, adapted macroeconomic policies, institutional reform, and redistributive policies are required. Inclusion is key, and preventive action needs to adopt a more people-centered approach that includes mainstreaming citizen engagement. Enhancing the participation of women and youth in decision making is fundamental to sustaining peace, as well as long-term policies to address the aspirations of women and young people.

Before new interventions are released into disease control programmes, it is essential that they are carefully evaluated in field trials'. Over the last twenty years the world has witnessed four major genocides. There was the genocide in Iraq (1988), in Rwanda (1994), in Srebrenica (1995), and in Darfur (2003 and continuing). Most observers agree there is an urgent need to assess the international community's efforts to prevent genocide and to intervene (once a genocide is under way) in an effective and timely manner. This volume, the latest in a widely respected series on the subject of genocide, provides an overview of a host of issues germane to this task. The book begins with a cogent discussion of the issues of prevention and intervention during the Cold War years. The second chapter discusses the abject failures and moderate (though, in some cases, highly controversial) successes at prevention and intervention carried out in the 1990s and early 2000s. Further chapters examine latest efforts to develop an effective genocide early warning system and examine the complexity of and barriers to prevention. The pros and cons of sanctions and the problems of enforcement and evaluation their effectiveness are then discussed. Conflicts between state sovereignty and the protection of threatened populations are examined both in historical context and by incorporating the latest thinking. Later chapters treat the issue of intervention; why and how it has met with only limited success. Concentrating on Rwanda and Srebrenica, chapter 8 discusses various peace operations that were abject failures and those that were moderately successful. The concept of an anti-genocide regime is examined in terms of progress in developing such a regime as well as what the international community must do in order to implement it. Chapters discuss key issues related to post-genocidal periods, those that need to be addressed in order to establish stability in a wounded land and populace as well as to prevent future genocides. During the past century the major causes of morbidity and mortality in the United States have shifted from those related to communicable diseases to those due to chronic diseases. Just as the major causes of morbidity and mortality have changed, so too has the understanding of health and what makes people healthy or ill. Research has documented the importance of the social determinants of health (for example, socioeconomic status and education) that affect health directly as well as through their impact on other health determinants such as risk factors. Targeting interventions toward the conditions associated with today's challenges to living a healthy life requires an increased emphasis on the factors that affect the current cause of morbidity and mortality, factors such as the social determinants of health. Many community-based prevention interventions target such conditions. Community-based prevention interventions offer three distinct strengths. First, because the intervention is implemented population-wide it is inclusive and not dependent on access to a health care system. Second, by directing strategies at an entire population an intervention can reach individuals at all levels of risk. And finally, some lifestyle and behavioral risk factors are shaped by conditions not under an individual's control. For example, encouraging an individual to eat healthy food when none is accessible undermines the potential for successful behavioral change. Community-based prevention interventions can be designed to affect environmental and social conditions that are out of the reach of clinical services. Four foundations - the California Endowment, the de Beaumont Foundation, the W.K. Kellogg Foundation, and the Robert Wood Johnson Foundation - asked the Institute of Medicine to convene an expert committee to develop a framework for assessing the value of community-based, non-clinical prevention policies and wellness strategies, especially those targeting the prevention of long-term, chronic diseases. The charge to the committee was to define community-based, non-clinical prevention policy and wellness strategies; define the value for community-based, non-clinical prevention policies and wellness strategies; and analyze current frameworks used to assess the value of community-based, non-clinical prevention policies and wellness strategies, including the methodologies and measures used and the short- and long-term impacts of such prevention policy and wellness strategies on health care spending and public health. An Integrated Framework for Assessing the Value of Community-Based Prevention summarizes the committee's findings.
