This report considers the biological and behavioral mechanisms that may underlie the pathogenicity of tobacco smoke. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

The book covers the three largest sources of particulate matter pollution in five chapters. These sources constitute three of the top ten public health problems in the world today and far outstrip any other environmental health threats in terms of health impact. The book begins with indoor solid fuel combustion for cooking in lower income countries and tells the story of how this problem was identified and recent efforts to eliminate it. The book next looks at tobacco smoking and second hand smoke, again reviewing the history of how these problems were identified scientifically and the fierce industry push back against the science. This book is the sixth volume in the series Acute Exposure Guideline Levels for Selected Airborne Chemicals, and includes AEGLs for chemicals such as ammonia, nickel carbonyl and phosphine, among others. At the request of the Department of Defense, the National Research Council has reviewed the relevant scientific literature compiled by an expert panel and established Acute Exposure Guideline Levels (AEGLs) for 12 new chemicals. AEGLs represent exposure levels below which adverse health effects are not likely to occur and are useful in responding to emergencies such as accidental or intentional chemical releases in the community, the workplace, transportation, the military, and for the remediation of contaminated sites. Three AEGLs are approved for each chemical, representing exposure levels that result in: 1) notable but reversible discomfort; 2) long-lasting health effects; and 3) life-threatening health impacts.
