Originally published in 2005. The prevention, detection and treatment of cancer has received enormous scientific and clinical attention in the US and in other developed countries. However, there has been no comprehensive review of the racial/ethnic disparities in cancer among elders, nor the opportunities for cancer prevention within the Medicare population. In this important work, John A. Capitman, Sarita Bhalotra and Mathilda Ruwe address this deficiency. The evidence report summarized in this book offers systematic syntheses of prior published research and qualitative assessments of emerging approaches in order to illustrate and clarify some of the debates surrounding cancer disparities. The past year was the second year of our intervention study that aims to improve the breast screening behavior among single (windowed, divorced, separated or never-married) African-American women ages 65 and older. During the period, we successfully (1) finished pre-intervention interviews with a high participation rate of eligible women, (2) finished interventions on the study subjects in the intervention group, (3) mailed intervention materials to significant others of the women in the intervention group, (4) did preparatory work for post-intervention interviews, (5) cleaned, edited, and analyzed pre-intervention data and (6) had some re-intervention results which will be presented at a scientific meeting. We are currently conducting post-intervention interviews. Our research team members and lay health educators ave done an outstanding job to reach the goal described in the statement of work.

The DOD funded HBCU/MI Partnership Training award entitled "Breast Cancer in African American Women: Molecular Analysis of Differences in Incidence and Outcomes" continues to provide quality research experiences to faculty and students and has developed into a highly rewarding collaboration that has expanded from the original 2 faculty at HUCC to include 6 experienced faculty and multiple students. At WRAIR, the original 2 has increased to include 3 new researchers and numerous support staff and students. We have had a total of 9 animal handling workshops attended by students and faculty interested in cancer research, several workshops on establishment of the various stages (virgin, pregnant and lactating) of normal breast organ cultures, workshop on imaging of live animals using luminescent-tagged cells, as well as small group collaborations/meetings on genomics, bioinformatics and proteomics. As a result of participating in our various workshops sponsored by this training grant, especially the in vivo imaging of live animals, Dr. Paul Wang was successful in receiving an HBCU-MI partnership grant from CDMRP-BCRP aimed at bringing this technology to Howard University. These workshops have been very popular with faculty and students performing both cancer and other basic science research. Additionally, our previous reports have detailed the presentation of four workshops on Grantsmanship, Grant Writing and Writing Successful Animal Protocols. We have amassed a multidisciplinary core of students, post-doctoral fellows and faculty who are poised to perform several collaborative studies on breast, and other cancers, as a result of training received from this program. We have jointly produced manuscripts yearly, made key presentations at conferences, published numerous abstracts and continue to expand the original specific aims of the proposal.

This genome-wide association of breast cancer includes over 3,000 African American women with invasive disease and over 3,000 age-matched African American controls from many existing case-control studies of breast cancer in the U.S. In the first year of this project all DNA samples from these studies were sent to Dr. Haiman's laboratory at the University of Southern California, quantitated and arrayed for genotyping. We have also assembled the covariate file that contains established breast cancer risk factor data collected from these studies, and the data have been standardized for the analysis. As of April 1, 2009, we have genome-wide scanned 2,200 cases and 2,200 controls from a number of the participating studies using the Illumina 1M Beadchip. We have assessed the blinded quality control replicates and overall call rates by SNP and sample and the data appear to be of very high quality. We have conducted a very preliminary statistical analysis of the data and there are a number of promising signals including what we believe is a novel risk locus for breast cancer that may be of particular importance for estrogen receptor negative breast cancer. The analysis will continue as additional samples/studies are scanned over the next year.

Compared to other subtypes of breast cancer, triple-negative breast cancer (TNBC) accounts for a disproportionate number of metastatic cases and cancer deaths. Glaring disparities are present in the occurrence of TNBC, such that those diagnosed are more likely to be African American (prevalence of 26% vs. 16% in non-African Americans) and premenopausal (24% vs. 15% postmenopausal). A critical factor to consider regarding the disparities associated with TNBC is the evidence documenting the link between psychosocial stress over the life course and the occurrence of large, aggressive tumors that are characteristic of this subtype. Because issues such as crime, isolation, stress, discrimination, and other factors associated with poverty have been found to significantly affect etiology of breast cancer among young, African American women, it is critical to also consider these issues after diagnosis occurs. Despite the fact that many of these factors impact the etiology of TNBC, little is known about how these factors come into play once the diagnosis has occurred. The present study qualitatively explores the critical biopsychosocial history and current environment of women facing such a diagnosis in order to shed light on the experience of TNBC. Using a grounded theory approach, in-depth, qualitative interviews were conducted with six women with TNBC and with a comparison group of six women with ER+ breast cancer. A prospective, longitudinal design was used with all women in the study to assess change over time and to cultivate prolonged engagement with participants. Data collection occurred in three waves, which corresponded with three critical points of the cancer care trajectory. Findings from this study demonstrate that the following stressors and strengths were unique for women with TNBC as compared to women with ER+ breast cancer: burden carriers throughout the life course, distant/strained relationships with mothers, absent fathers, experiences of sudden, unexpected deaths of loved ones, limited engagement in a spiritual community, limited engagement in neighborhood/community, and young age at time of diagnosis. Findings from these interviews resulted in a preliminary conceptual framework for understanding the contextual lives of women with TNBC, which can assist with guiding the formation of appropriate social work interventions.

Data collection for a population-based case-control study of breast cancer conducted in the San Francisco Bay Area has been on-going since July 1996. Cases include African-American and White women aged 35-79 years and diagnosed with breast cancer between 1995 and 1998. Controls are identified through random-digit dialing. Information on physical activity, sunlight exposure, dietary intake of vitamin D and phytoestrogens, and other risk factors, is collected by in- person interview. The home visit also includes measurements of anthropometry and skin pigmentation using a Chromameter. To date, 607 case and 736 control interviews have been completed. Data from this study will be pooled with the data collected in two on-going case-control studies in African-American, Latina, and White women that use the same methodology and questionnaire. The combined data for an estimated 1300 cases and 1650 controls will allow us to examine the risk factor profile among White (high risk), African-American (moderate risk), and Latina (low risk) women. This study will make an important contribution to the sparse epidemiologic literature on breast cancer risk factors in African-American and Latina women.
