Health care in America is plagued by extensive gaps in quality. Too often care provided to patients does not match what the medical community has determined to be the most effective care. Abundant research has shown that these gaps in quality are responsible for increased costs, wasteful and ineffective care, preventable complications, avoidable hospitalizations, decreased quality of life, disability, and premature death. The National Healthcare Quality Report (NHQR) and National Healthcare Disparities Report (NHDR), published annually by the Agency for Healthcare Research and Quality since 2003, provide both extensive research and data on the extent of health care quality gaps as well as national benchmarks for quality. This Resource Guide and its accompanying Workbook draw on the NHQR and the NHDR to support State-level efforts to improve the quality of asthma care. This Resource Guide is designed to help State leaders identify measures of asthma care quality, assemble data on asthma care, assess areas of care most in need of improvement, and learn what other States have done to improve asthma care. Taken together, the Resource Guide and its companion Workbook can help State leaders to develop an asthma quality improvement action strategy. Asthma is a chronic lung condition that impairs normal breathing. The disease affects a growing number of Americans. In 2003, nearly 30 million people had been diagnosed with asthma at some point in their lives and nearly 20 million people stated they currently had asthma (Centers for Disease Control and Prevention, 2002a). Asthma is also costly: total estimated costs in 2001 were $14 billion (American Lung Association, 2004). For several reasons, asthma presents an opportune target for quality improvement: Increased prevalence, especially among children and adolescents; Disparities between socioeconomic groups and between racial/ethnic groups in terms of diagnoses and quality of asthma care; A range of interventions and treatment that can successfully control the disease and prevent attacks; High health care costs of uncontrolled asthma and the potential for a positive return on investment for purchasers and the health care system as a whole through asthma quality improvement. Improved quality of asthma care may help to cut costs, reduce disparities, and improve the quality of life for millions of people with asthma.

One-liner: A set of policy recommendations to promote the development and maintenance of communities in which children with asthma can be swiftly diagnosed, effectively treated, and protected from exposure to harmful environmental factors. An estimated 5 million U.S. children have asthma. Too many of these children are unnecessarily impaired. Much of the money spent on asthma is for high-cost health care services to treat acute periods of illness. Many asthma attacks could be avoided--and much suffering prevented and many medical costs saved--if more children received good-quality, ongoing asthma care and if the 11 policy recommendations presented in this report were implemented in a oordinated fashion. A national call to action, the policy recommendations span public and private interests and compel integration of public health activities across local, state, and federal levels. This report summarizes the findings of an effort funded by the Robert Wood Johnson Foundation as part of the Pediatric Asthma Initiative, whose purpose is to address current gaps in national childhood asthma care. It is the first national initiative that simultaneously addresses treatment, policy, and financing issues for children with asthma at the patient, provider, and institutional levels. The purpose of RAND's effort was to:--identify a range of policy actions in both the public and private sectors that could improve childhood asthma outcomes nationwide--select a subset of policies to create a blueprint for national policy in this area--outline alternatives to implement these policies that build on prior efforts.The effort developed a comprehensive policy framework that maps the identified strategies to one overall policy objective: to promote the development and maintenance of asthma-friendly communities--communities in which children with asthma are swiftly diagnosed, receive appropriate and ongoing treatment, and are not exposed to environmental factors that exacerbate their condition. Asthma is one of the most common chronic medical conditions in the U.S. It affects 16 million adults and 6.1 million children and results in two million visits to emergency departments, 70,000 hospitalizations, and 5,000 deaths annually. The burden of asthma disproportionately affects persons of lower socioeconomic status. Because asthma can neither be prevented nor cured, current management objectives are to monitor symptoms and objective measures of lung function, to encourage the use of medications that control and prevent symptoms with the fewest adverse effects possible, to control the triggers of asthma symptoms to which a patient is sensitive (such as house dust mites, tobacco smoke, animal dander, and pollens), and to facilitate an asthma care partnership between patients and providers. Effective asthma management has been demonstrated to reduce symptoms, hospitalizations, and urgent care visits. Despite the availability of evidence-based guidelines for the management of pediatric and adult asthma, there remains a significant gap between accepted best practices for asthma care and actual care delivered to asthma patients. The objective of this systematic review was to evaluate the evidence that quality improvement (QI) strategies can improve the processes and outcomes of outpatient care for children and adults with asthma, and to identify the most effective strategies. The two research questions addressed in this study were: Research Question 1: What is the evidence that QI strategies improve the processes and outcomes of outpatient care for pediatric and adult populations with asthma? Specifically, which QI strategies are effective for improving processes and outcomes of asthma care for specific patient populations (e.g., adults, children, low socio-economic status (SES), racial groups, urban/rural)? Also, does the setting of the QI intervention (e.g., home, school, clinic) determine its effectiveness for improving processes and outcomes of asthma care? Research Question 2: Are QI interventions for asthma care that incorporate multiple strategies more effective than those that employ a single strategy?

East Tennessee faces many challenges with adult asthma management. Summit Medical Group (Summit), the largest organization providing primary care in the East Tennessee region, finds asthma management challenging due to inconsistent care from primary care physicians (PCPs), pulmonologists, and health coaches. Certain essentials to asthma care such as performing pulmonary function tests and educating patients on disease and medications are not well-coordinated and vary by provider. In addition to these challenges, the Asthma and Allergy Foundation of America ranks Knoxville as the second most challenging city in America for people with asthma to live. Therefore, with funding from AstraZeneca, the Public Health Foundation (PHF) selected Knoxville as the site of a quality improvement (QI) demonstration project.
