INTRODUCTION. Malgré les progrès médicaux, l'arthroplastie totale du genou (PTG) reste l'un des traitements de référence dans les arthropathies de la polyarthrite rhumatoïde (PR). L'objectif de cette étude était de comparer les résultats radio-cliniques de deux séries de PTG : PR et arthrose. MATERIEL ET METHODE. Une étude monocentrique rétrospective a été réalisée dans le service de chirurgie orthopédique du CHU H. Mondor de Créteil de août 2006 à juin 2016 incluant 107 patients (127 PTG) - 75 dans le groupes gonarthrose (82 PTG) et 32 dans le groupes PR (45 PTG). Le critère de jugement principal était l'analyse de l'angle HKA (hip-knee-ankle) en péri-opératoire. Les critères de jugements secondaires étaient l'évaluation clinique, les difficultés peropératoires et le taux de complications. RESULTATS. L'angle HKA moyen en postopératoire était de 179° (180° dans le groupe PR vs. 178,5° dans le groupe gonarthrose), sans différence significative malgré un HKA préopératoire significativement différent. L'alignement frontal a été optimal (HKA = 180 ± 3°) pour 77,5 % des PR vs. 71,5 % des gonarthroses. L'IKS au dernier recul (2 ans) était de 149,5 pour le groupe PR vs. 160 pour le groupe gonarthrose (P=0,048). Il n'existait pas de différence significative en termes de complications. En analyse multivariée, dans le groupe PR, un HKA>190° en préopératoire était associé à un HKA postopératoire plus élevé (OR IC 95% 1,56 (1,10 - 2,20) ; P= 0,011). CONCLUSION. La correction de l'axe ne diffère pas entre les deux groupes mais les défauts de correction semblent être associé aux grandes déformations. Les patients ayant un PR ont un score fonctionnel inférieur à celui de la population générale.
