This textbook examines the ways in which arguments may be used and abused in medicine and health. The central claim is that a group of arguments known as the informal fallacies – including slippery slope arguments, fear appeal, and the argument from ignorance – undertake considerable work in medical and health contexts, and that they can in fact be rationally warranted ways of understanding complex topics, contrary to the views of many earlier philosophers and logicians. This chapter is concerned with the reasons why sometimes good arguments in health communication leaflets fail to convince the targeted audience. As an illustrative example it uses the age-dependent eligibility of women in the Netherlands to receive routine breast cancer screening examinations: according to Dutch regulations women under 50 are ineligible for them. The present qualitative study rests on and complements three experimental studies on the persuasiveness of mammography information leaflets; it uses interviews to elucidate reasons why the arguments in the health communication leaflet.

But this is not a simple matter of rich and poor. A poor man in Glasgow is rich compared to the average Indian, but the Glaswegian's life expectancy is 8 years shorter. These approaches only go so far. Creating the conditions for people to lead flourishing lives, and thus empowering individuals and communities, is key to reduction of health inequalities. In addition to the scale of material success, your position in the social hierarchy also directly affects your health, the higher you are on the social scale, the longer you will live and the better your health will be. As people change rank, so their health risk changes. What makes these health inequalities unjust is that evidence from round the world shows we know what to do to make them smaller. This new evidence is compelling. It has the potential to change radically the way we think about health, and indeed society.

Health care reform is always about more than morality and values, but if it lacks a moral foundation, it will not stand the test of time. Dr. Olson provides a moral foundation expressed in eight standards derived from his study of ethics, systems theory, and health policy research.
