Actuellement, près de 10 % des femmes enceintes sont porteuses d'un utérus cicatriciel. L'épreuve du travail, en cas d'utérus cicatriciel, est dominée par le risque de rupture utérine. La solidité de la cicatrice utérine est un des critères majeurs qui doit orienter le choix du praticien pour le mode d'accouchement. Le segment inférieur ne se formant qu'au cours du troisième trimestre, les césariennes pratiquées avant 32 semaines d'aménorrhée (SA) pourraient être associées à un excès de risque de rupture utérine par rapport aux césariennes réalisées à terme. Nous avons mené une étude cherchant à estimer le risque de rupture utérine en cas de tentative d'accouchement par voie vaginale chez des femmes ayant un antécédent de césarienne avant 32 SA (cas). Cent huit tentatives d'accouchement par voie basse chez des patientes ayant un antécédent de césarienne avant 32 SA ont été étudiées et comparées à 137 épreuves du travail chez des patientes ayant un antécédent de césarienne après 32 SA. Trois ruptures utérines sont survenues dans le groupe des cas soit 2,8%, IC95% [0-3,8%], et quatre dans celui des témoins soit 2,9% (p=0,9). Ces ruptures utérines n'ont pas entraîné de complication materno-fœtale grave. L'état néonatal était comparable dans les 2 groupes. Compte tenu du faible nombre de rupture utérine et de l'absence de complication observée, sur une période de huit années, il nous semble raisonnable, de proposer un accouchement par voie basse aux femmes ayant un antécédent de césarienne avant 32 SA. Ces données nécessitent d'être confirmées par des séries plus larges

52 BC is the key year of the Gallic Revolt, with the near-disastrous Roman defeat at Gergovia followed by the climactic victory over the Gallic chieftain Vercingetorix at Alesia. In 52, BC Caesar's continued strategy of annihilation had engendered a spirit of desperation, which detonated into a revolt of Gallic tribes under the leadership of the charismatic young Arvernian noble Vercingetorix. Major engagements were fought at Noviodunum, Avaricum, and Gergovia, with the last action being the most serious reverse that Caesar faced in the whole of the Gallic War. However, Vercingetorix soon realized that he was unable to match the Romans in pitched battle. Taking advantage of the tribesmen's superior knowledge of their home territory, Vercingetorix began a canny policy of small war and defensive manoeuvres, which gravely hampered Caesar's movements by cutting off his supplies. For Caesar it was to be a grim summertime – his whole Gallic enterprise faced disaster. In the event, by brilliant leadership, force of arms, and occasionally sheer luck, Caesar succeeded in stamping out the revolt in a long and brutal action culminating in the siege of Alesia. Vercingetorix finally surrendered and Alesia was to be the last significant resistance to the Roman will. Never again would a Gallic warlord independent of Rome hold sway over the Celts of Gaul.

In Socialism of Fools, Michele Battini focuses on the critical moment during the Enlightenment in which anti-Jewish stereotypes morphed into a sophisticated, modern social anti-Semitism. He recovers the potent anti-Jewish, anticapitalist propaganda that cemented the idea of a Jewish conspiracy in the European mind and connects it to the atrocities that characterized the Jewish experience in the nineteenth and twentieth centuries. Having a child remains one of the biggest health risks for women worldwide. Fifteen hundred women die every day while giving birth. That's a half a million mothers every year. UNICEF's flagship publication, The State of the World's Children 2009, addresses maternal mortality, one of the most intractable problems for development work.The difference in pregnancy risk between women in developing countries and their peers in the industrialised world is often termed the greatest health divide in the world. A woman in Niger has a one in seven chance of dying during the course of her lifetime from complications during pregnancy or delivery. That's in stark contrast to the risk for mothers in America, where it's one in 4,800 or in Ireland, where it's just one in 48,000. Addressing that gap is a multidisciplinary challenge, requiring an emphasis on education, human resources, community involvement and social equality. At a minimum, women must be guaranteed antenatal care, skilled birth attendants and emergency obstetrics, and postpartum care. These essential interventions will only be guaranteed within the context of improved education and the abolition of discrimination.
