Unilateral vocal fold paralysis is a condition that can cause significant vocal dysfunction. Treatment options include behavioural therapy or surgical procedures. The purpose of the present study was to investigate specific vocal function parameters in subjects with a compensated unilateral vocal fold paralysis, and to investigate the effects of medialisation thyroplasty on these parameters. The pathophysiology of the voice abnormality associated with a unilateral vocal fold paralysis and the relevant literature regarding treatment has been reviewed. Measurements included indirect laryngoscopy, acoustic data (fundamental frequency, percent jitter and shimmer), fundamental frequency and intensity range, voice range profile, phonation threshold pressure, perceptual evaluation and a reading task. Results indicated that most subjects exhibited a reduced fundamental frequency and intensity range compared to normal values prior to surgery. A comparison of data from before and after medialisation thyroplasty indicated that most subjects showed improvements in the vocal parameters evaluated.

Many patients with unilateral vocal fold paralysis cannot attain satisfactory vocal function without surgical management. The current investigation examined pre- and postoperative measures of laryngeal function in groups of individuals who had undergone either Teflon injection, thyroplasty type 1 or a combination of thyroplasty plus secondary Teflon injection. This latter group consisted of patients who had experienced unsatisfactory results following the initial thyroplasty procedure. Four sets of vocal parameters were analyzed: acoustic, aerodynamic, videostroboscopic and auditory perceptual. Significant improvements were noted for airflow rates and maximum intensity level. Changes in jitter, shimmer, signal-to-noise ratio, maximum phonation time, and ratings of breathiness and voice quality approached significance. Postoperative measures rarely reached normal levels. Although all three groups exhibited some change following treatment, the thyroplasty group consistently exhibited the greatest improvement in the variables examined. It was concluded that although successful thyroplasty procedures demonstrate superior postoperative results, vocal function following unsuccessful procedures can be augmented or salvaged with secondary Teflon injection.
