Les AINS, malgré une grande diversité chimique, ont la propriété commune de bloquer la cyclo-oxygénase (COX), enzyme responsable de la formation de prostaglandines. Celles-ci, outre leur rôle pro-inflammatoire, ont un rôle physiologique important et leur inhibition est à l'origine de la plupart des effets indésirables des AINS. Ceux-ci sont principalement digestifs et rénaux mais également hématologiques, hépatiques et génitaux. Il faut également souligner la toxicité potentielle des AINS et des antalgiques en particulier chez le chat, qui du fait d'une glucuronoconjugaison déficiente, est extrêmement sensible au paracétamol et à l'acide flufénamique. La découverte de 2 isoformes de la cyclo-oxygénase (COX1 : synthèse de prostaglandines à rôles physiologiques et COX2 : synthèse de prostaglandines à rôles pro-inflammatoires), a permis le développement de molécules inhibant préférentiellement (voire uniquement) la COX2 et dont les effets indésirables sont réduits. Parallèlement, la découverte de substances, les "doubles inhibiteurs", capables d'inhiber à la fois la COX et la 5-LOX (responsable de la formation de leucotriènes, pro-inflammatoires), est à l'origine du développement de nouveaux AINS qui semblent allier efficacité et grande sécurité d'emploi. Une bonne connaissance du mode d'action associée à un emploi raisonné de ces molécules va permettre une utilisation plus sécurisante dans de nombreuses indications : affections ostéo-articulaires, chirurgie, pneumologie, ophtalmologie.
