L'objectif de ce mémoire est de créer un matériel de rééducation des troubles prosodiques expressifs, destiné aux enfants porteurs de trisomie 21 âgés de 6 à 12 ans. La trisomie 21 est la pathologie génétique la plus fréquente. Elle entraîne un morphotype et un phénotype caractéristiques avec, entre autres, des troubles d'origine neuro-centrale. Des troubles de la communication sont également retrouvés chez ces enfants : leur parole est peu intelligible. Nos recherches ont mis en exergue que cette inintelligibilité pouvait être due, en partie, à des difficultés prosodiques présentes sur le versant expressif et peu prises en charge par les orthophonistes. Nous avons passé en revue les matériels de rééducation existants pour ce type de troubles. Aucun ne nous a semblé totalement adapté à la trisomie 21 : soit ils ne permettent pas de travailler les difficultés prosodiques rencontrées spécifiquement dans cette pathologie, soit ils ne prennent pas en compte les troubles cognitifs, perceptifs et moteurs. Face à ce constat, nous avons imaginé un matériel ciblant tout particulièrement les difficultés prosodiques des enfants porteurs de trisomie 21 et prenant en compte leurs difficultés autres. Nous avons donc créé un outil, composé de plusieurs jeux, qui permet une progression par « étapes » dans la rééducation. Le travail porte tout d'abord sur la gestion du souffle phonatoire et sur l'enchaînement des mouvements bucco-faciaux. Les exercices se centrent ensuite sur la modulation de la fréquence, de l'intensité et de la durée. Des pistes sont enfin données aux orthophonistes pour permettre une généralisation dans la communication quotidienne de l'enfant porteur de trisomie 21. L'orthophoniste avance ainsi pas à pas dans la prise en charge en ciblant les déficits spécifiques de l'enfant. Nous voulions que le matériel soit ludique, mais également très adaptable pour couvrir la population-cible la plus large possible. Des idées d'adaptation sont ainsi proposées à l'orthophoniste pour chaque exercice.

Between 2000 and 2010, there was an unprecedented rise in the number of applications and the number of children found to meet the disability criteria. The factors that contribute to these changes are a primary focus of this report. Speech and Language Disorders in Children provides an overview of the current status of the diagnosis and treatment of speech and language disorders and levels of impairment in the U.S. population under age 18. This study identifies past and current trends in the prevalence and persistence of speech disorders and language disorders for the general U.S. population under age 18 and compares those trends to trends in the SSI childhood disability population.
