Malgré le dépistage néonatal de la luxation congénitale de la hanche, certains cas sont encore diagnostiqués après l’âge d’acquisition de la marche. Le traitement de ces hanches est alors très controversé, entre réduction orthopédique ou chirurgicale. Ce travail rapporte les résultats de la méthode de réduction orthopédique progressive avec traction selon les principes de Petit-Morel chez des enfants traités entre l’âge de 1 et de 5 ans. La série est composée de 72 hanches traitées entre 1975 et 2000. La traction est appliquée horizontalement, dans l’axe du membre puis en abduction et rotation interne, ce qui permet à la tête fémorale d’être "présentée". Le plâtre qui suit va permettre la «pénétration» de la tête et sa stabilisation dans le cotyle. Pour beaucoup d’enfants, une ostéotomie pelvienne a été nécessaire pour corriger la dysplasie résiduelle. Les résultats évaluent la congruence coxo-fémorale (Severin) et la présence d’une ostéochondrite post-réductionnelle (Kalamchi et MacEwen). Pour 95,8% des hanches, le résultat était qualifié de bon ou très bon au recul final. Pour 2 cas, il existe un échec de la réduction orthopédique avec reluxation. Le seul cas d'ostéochondrite est survenu pour une de ces 2 hanches. En comparant nos résultats à ceux de la littérature, il n’existe pas de différence franche entre les différentes techniques en terme de congruence coxo-fémorale (Severin). Par contre, la réduction progressive sans manœuvre de réduction manuelle de cette méthode semble être la plus efficace pour prévenir l’ostéochondrite. La méthode décrite dans cet article semble donc être celle de choix pour le traitement de la luxation congénitale de la hanche de l’enfant entre 1 et 5 ans.

Il s'agit d'une série continue restrospective de 43 enfants (40 filles et 3 garçons) vus à l'âge de la marche pour une luxation vierge de tout traitement préalable. Cette série comportait 57 hanches (50 luxations, 7 sub-luxations). L'âge moyen au début du traitement était de 18,6 mois (extrême 10 et 36 mois). Le traitement a consisté en une traction continue avec abduction progressive suivie d'une immobilisation plâtrée (de 143 jours en moyenne) et d'une ostéotomie de correction (ostéotomie de Salter pour 52 hanches et association à une ostéotomie fémorale pour 8 hanches). Les dossiers ont été analysés avec un recul moyen de 12 ans par rapport au début du traitement (extrêmes 10 mois et 15 ans). Les résultats précoces montrent que les hanches ont été recentrées dans 55 cas sur 57. Parmi ces 2 échecs, l'un a été opéré par réduction chirurgicale et l'autre a été négligé. Pour l'ensemble de la série, la remise en charge a été effectuée en moyenne au bout de 11 mois (extrême 6 mois et 18 mois). Des complications ont été observées chez 5 enfants : 4 reluxations (ayant nécessité une ostéotomie fémorale associée 2 fois à une capsuloraphie), 1 sepsis superficiel après ostéotomie du bassin et 3 ostéochondrites dont 2 mineures...

Although the number of published papers on traumatic hip disloca tion in childhood is large, our actual current knowledge on this inju ry in this age group is still full of obscure points and unsolved prob lems. Luxation of the hip of traumatic origin in children is infrequent, and the small number of cases reported by authors and institutions, as well as the usually short follow-up in many cases, are responsible for the many doubts and questions, especially related to complications and outcome following the injury. Consequently the treatment, especially the postreduction management, has not been adequately defined. Dr. Antonio Barquet has made the great effort to analyze the ex perience gained from 38 cases in Uruguay and to review more than 500 references related to the subject, covering more than 1400 cases. He began to study this topic in 1976. It was the subject of his postgraduate monograph in 1979, which was followed by a statisti cal analysis of the major complication, i. e., avascular necrosis of the proximal femoral extremity, which was presented as his doctoral thesis in 1981. In addition, he has published numerous papers deal ing with several aspects of traumatic hip dislocation in children.

This book is concerned with the effect that displacement, whether minimal or severe, may have on the hip joint. Although it is concerned with the changes which take place in childhood and during growth, when they are most common and most severe, it is also to a lesser extent concerned with the way they will continue or even start long after growth has ceased. It is based on a series of about 450 cases of congenital displacement of the hip treated when the deformity was established, together with unstable hips drawn from 82000 children whose hips were examined at the time of birth. This study was carried out at the Nuffield Orthopaedic Centre in Oxford. It was started in early 1949 by the author with Mr. J. C. Scott and continued until mid-1977, since when it has continued in the capable hands of Mr. J. W. Goodfellow and Mr. M. K. Benson. The study was started at a time when the generally accepted view was still that the displacement was part of the primary failure of development of the acetabulum, which could not adequately contain the femoral head. Conservative treatment with manipulative reduc tion followed by a prolonged period of plaster immobilisation was the method of choice. Few attempts had been made with surgery as a primary procedure and these had not met with continuing success.
