Thyroid hormone plays an important role in development and functional maintenance in the central nervous system. Deficiency of thyroid hormone during fetal and early postnatal life induces abnormal development known as cretinism in humans. However, the molecular mechanism of thyroid hormone action has not yet been fully understood. Thyroid hormone action in the brain may be disrupted under various pathological conditions. In addition, environmental factors including endocrine-disrupting chemicals and bacterial endotoxins may disrupt thyroid hormone action in brain, causing abnormal brain development and functional disruption. This is a first book to comprehensively describe the effect of thyroid hormone disruption in the central nervous system. The first section deals with the disruption of thyroid hormone action at the molecular level. First the authors provide a summary of the possible molecular mechanisms of thyroid hormone action in the brain, then they discuss several factors that may disrupt thyroid hormone action. In the second section, animal models to study thyroid hormone action will be introduced. An interesting character of thyroid hormone deficiency is that, without thyroid hormone, the thyroid hormone receptor may act as a “repressor” of gene expression, causing more severe consequence than those of thyroid hormone receptor knockout animals. Thus, several different kind of animal models may be used to clarify the role of thyroid hormone and its receptor in the brain. In the third section, human studies on thyroid disease and neurodevelopment will be introduced. Although endemic cretinism induced by iodine deficiency and sporadic cretinism by various thyroid mutation are well known, the pathophysiological mechanisms that create each abnormal phenotype are not fully understood.

Both thyroid dysfunction and heart failure show a high prevalence in the adult population. Frequently, in clinical practice, a multidisciplinary approach is useful to optimize the management of patients with these conditions. Although there is no doubt regarding the close link between cardiovascular pathophysiology and thyroid homeostasis, our understanding of this association is far from being exhaustive. Thyroid hormone regulates the expression of cardiac-specific functional contractile and structural proteins and plays a pivotal role in modulating both diastolic and systolic function as well as peripheral vascular resistance. The close relationship between thyroid and heart dysfunction is strongly supported by recent evidence demonstrating that an altered thyroid profile is a negative prognostic predictor in patients with heart failure. The treatment of chronic heart failure, especially in advanced stages of the disease, continues to be an open and challenging field. The Second International CBT (Center for Biotechnology) Synposium was held in Stockholm November 4-5 1988 and had the title "The steroid/thyroid hoITIDne receptor family and gene regulation". This meeting contained contributions from most leading laboratories within the field of steroid/thyroid hOITIDne receptors and gave a very exciting perspective on the dynamic development of this i:ntx>rt. ant research field. Today all known steroid hoITIDne receptors have been cloned and sequenced and novel members of this supergene family are constantly being discovered. Sane of these new receptors remain to be characterized regarding the nature of their ligand. Access to probes for steroid receptors has enabled studies on mechanisms of regulation of receptor gene expression. Deletion and II'Altational analysis of steroid receptor cDNAs followed by expression in cells together with suitable reporter genes has yielded a detailed knowledge about the functional significance of the various domains the receptors are composed of. In certain cases, steroid resistance in patients have been shown to be due to point mutations in the corresponding steroid receptor genes resulting in non-functional receptors. The availability of receptor cDNAs also makes it possible to express receptors at high levels in procaryotic and eucaryotic cells. It is e. g. possible to express the DNA-binding domain of the glucocorticoid receptor in E. coli as a fusion protein with protein A which interacts specifically with DNA. Such studies are necessary for production of sufficient quantities of receptors to allow crystallization and X-ray crystallography for detailed structural information.
