The common and unfortunate complication after cardiac arrest is hypoxic-brain injury, which leads to devastating neurological complication. There are only 7% to 30% of the patient, discharged at home with good neurological outcome, who were admitted after cardiac arrest. The initial emergency treatment of stabilizing the heart and the intensive care unit (ICU) management to treat the underline cause of the cardiac arrest are still not resulting in improved neurological outcome or decrease mortality rate in post cardiac arrest patient. The induction of therapeutic hypothermia (TH) of 32 ?C ' 34 ?C in ICU within 3-4 hours for 24 hour and then rewarm slowly at temperature of 0.3- 0.5 ?C every hour, with injunction of the other care management and treatment for post cardiac arrest patient. Evidences has suggested that therapeutic hypothermia can provide cerebral neuro-protective benefits in post cardiac patients even with different methods of hypothermia induction. The treatment process to desired outcome requires close monitoring of the patient, competent nurses with knowledge and skills of the treatment. The therapeutic hypothermia is concurrently implemented in hospitals for the short term and long term patient quality patient care outcome. Since 2007, the year hypothermia was included in treatment plan for post cardiac arrest by different hospitals, the mortality rate of these hospital has significantly decreased and the neurologic disability also decreased among the survivors overtime.

Problem: Effective therapeutic hypothermic treatment involves quick and careful management in order to optimize neurological outcomes and improve the morbidity and mortality associated with cardiac arrest. Currently there is no standardized guideline in implementation of therapeutic hypothermia across institutions throughout the country. There are many studies that look into this treatment; however, the sample sizes have been small and many confounding variables have affected outcome. This literature review aims to look at recent studies involving different aspects of treatment such as target temperature of cooling, method of cooling, and investing new novel techniques in cooling treatment that are used to improve patient outcomes. Methods: The search for articles was performed using the Weill Cornell Medical Library to access PubMed using the following search terms: "therapeutic hypothermia" and "brain", "cardiac arrest" and "brain", "target temperature" and "therapeutic hypothermia", "methods of cooling in therapeutic hypothermia." Results: The search for "therapeutic hypothermia" resulted in 23,492 results while "therapeutic hypothermia cardiac arrest" yielded 4903. Many of the results were in - depth discussions about side effects, techniques of measuring cooling, and pediatric cases which were not as relevant to this review. A large amount of literature has been recently published as hypothermic treatment has been studied for many years; however, continued detailed research helps move its use into the spotlight. Overall 17 articles were used which detailed clinical trials in which hypothermia has been used, its outcomes, and effect on the brain. Conclusions: Though there have been many studies that have looked at the implementation of therapeutic hypothermia in cardiac arrest, there needs to be further evidence -based medicine in order to standardize treatment guidelines. Larger sample sizes should also be taken into account in order to capture a larger patient population that can benefit from these treatments. The goal of the following proposal will be to look into an area that has not been studied as extensively.

When the heart ceases to beat and carry the needed nutrition to the vital organs of the body a major concern is brain function. Just like with a stroke, cardiac arrest deprives the brain of the blood, oxygen and nutrients to stay viable, as the saying goes, Time is Brain. Knowing the steps both outside the hospital and inside the hospital will make the difference between life and death and whether or not the patient will lose all cognitive function. Rapid CPR, call to EMS, the hospital receiving the patient in a timely manner along with the patient being assessed for the use of hypothermia protocol are all key roles in the success of the patients outcome. Once the patient has been assessed and is a candidate acceptable for therapeutic hypothermia it is imperative that the patient has reached goal temperature within three hours of reaching the hospital along with continued close monitoring of temperature, vitals, electrolyte and overall functions. Patients should remain at goal temperature for twelve to twenty-four hours to give the body the time it needs to heal, recovery and hopefully allow the patient to regain all functions once they have completed the therapy or after extensive physical therapy.

After this is implemented, there would also need to be an in depth evaluation of the cooling process and the patient outcomes. Although this would be a long and tiring task to perform, the end result should absolutely be worth it.

There is an upheaval in evidence-based practice regarding the positive neurological outcomes in post cardiac arrest patients who have had therapeutic hypothermia initiated on them. Now that therapeutic hypothermia protocols are more commonly initiated in the Emergency Department, it makes it crucial for Emergency Department nurses to comprehend the evidence-based research behind using it, and the importance of early application in order to help improve neurological function in these post cardiac arrest patients. This paper discusses all phases of the project to educate the nursing and support staff of a local Emergency Department. The following paper chronicles the problem description, a solution description, the implementation plan, an evaluation plan, the dissemination plan, and a review of the literature that has been assessed. In addition, an exemplar of the pre and post educational survey as well as an exemplar of a therapeutic hypothermia brochure to be distributed is attached in the appendices for viewing. Fifteen peer-reviewed studies were thoroughly researched and critically appraised. The majority of the material researched included quantitative and qualitative research. The resulted findings were all significant to nursing and patient care as they have deliberated from these evidence based trials that inducing therapeutic hypothermia in post cardiac arrest patients is effective in optimizing patient outcomes by restoring neuronal function. A few of the evidence-based studies also resulted that therapeutic hypothermia is not being induced as soon as it should be due to barriers and knowledge deficit.
