[Truncated abstract] The low density lipoprotein receptor-related protein (LRP), a member of the low-density lipoprotein (LDL) receptor gene family is involved in numerous biological processes including lipoprotein metabolism. This thesis concerns investigations into some aspects of LRP metabolism/regulation and possible roles in coronary artery disease (CAD). Specific aims were: to investigate the association between polymorphisms in the LRP gene and in its associated protein, the lipoprotein receptor-associated protein (RAP), with the risk of CAD; to extensively examine the influence of the LRP exon 22 C200T polymorphism on lipid metabolism; to develop and characterise assays for the mRNA expression of LRP and 2 other genes relevant to lipid metabolism, the LDL receptor (LDLR), and HMG CoA reductase (HMGCR); and finally, to apply the latter techniques to studies on the influence of genetic variation in LRP, and dietary and drug interventions, on LRP, LDLR and HMGCR mRNA expression in nucleated blood cells from healthy human subjects. Six hundred CAD subjects and 700 similarly aged controls were genotyped for 8 LRP gene polymorphisms as well as for the RAP V311M polymorphism. ... In the final phase of my studies, I examined the influence of 4 weeks therapy with a cholesterol lowering drug, an HMGCR inhibitor, atorvastatin (20mg daily), on the mRNA expression of LDLR, LRP and HMGCR in human nucleated blood cells. Twelve normal Caucasian male subjects aged 49 ± 5 (SD) years were studied. Plasma total cholesterol and LDL-C decreased by averages of 29 % and 41 % after the 4 week period. This was accompanied by an elevation in LDLR mRNA expression by approximately 30 35 %. In contrast, there was no significant effect on LRP and HMGCR mRNA expression. In conclusion, the original findings in this thesis included: demonstration of a strong influence of the LRP exon 22 C200T polymorphism on coronary artery disease and LDLR expression, but without a clear effect on fasting or postprandial lipid levels; data on the biological variation in LDLR and LRP gene expression in nucleated blood cells from normal subjects; the influence of an oral fat load on the expression viii of these genes, finding that LDLR was significantly depressed; and finally, the observation that statin therapy upregulated LDLR in nucleated blood cells.

Profound mortality rates, due to cardiovascular disease, are a worldwide problem. Cardiovascular disease results from complications of a silent and chronic arterial disease: atherosclerosis. The challenge for the practitioner is adapting diagnostic and therapeutic responses to prevent this common and complex disease. Dyslipidemia, are disorders of the metabolism of soluble transporters of lipids in extracellular spaces of the human body (including blood), called lipoproteins. They are major cardiovascular risk factors, causally related with atherosclerosis and are themselves multifactorial diseases, resulting from interactions between genetic and environmental factors. The study of genetic factors has recently taken a new path with the study of DNA as an experimental object. More than fifty genes of lipoprotein metabolism have been identified in both their physiological actions and their contribution to the pathogenesis of human dyslipidemia. The diversity of observations has refined our current knowledge of the control of lipid metabolism and energy homeostasis in living organisms beyond the limits of the cardiovascular system (e.g., brain, immune system, and development). These studies have given way to a shake-up of former phenotypic classifications, distinguishing new entities, defining targeted therapeutic strategies, providing a basis for different patterns of disease distribution in human populations.

Low density lipoproteins (LDL) are pathophysiologically important be cause of their central role in the disease atherosclerosis and because atherosclerosis is the leading cause of death in developed countries. Many researchers believe that a more detailed knowledge of the struc ture, function, and metabolism of LDL may eventually lead to a means to control atherosclerosis. For this reason a fairly large research effort has gone into the investigation of LDL over the past few years. The purpose of this book is to collect and summarize in one place most of the pub lished information on LDL through 1975. To this end more than 1500 references are cited in the papers that make up this volume. The A, B, C apolipoprotein classification system was adopted for use throughout this work. In addition to the A, B, C, and "D" families of apolipoproteins, apoE is used to designate the "arginine-rich" apolipo protein. This classification system is used because it is far less cumber some than other proposed classification schemes for apolipoproteins.

Cholesterol is essential for normal cellular function. It is found in all cells of the body and is a major constituent of cell membranes. In addition, certain specialized tissues use cholesterol as the basic substrate for the synthesis of more complicated sterols such as bile acids and the steroid hormones. Like most lipids cholesterol has a very low solubility in water. Therefore, in order for these molecules to be transported be tween different organs of the body a series of lipid carriers known as lipoproteins has evolved. Over the last several years epidemiological evidence has accumulated suggest ing a strong relationship between the risk of atherosclerosis and high plasma concen trations of one of these lipoproteins, low density lipoprotein (LDL) 1). Thus, in popula tions with high plasma concentrations of LDL-cholesterol a correspondingly high incidence of vascular disease is seen. Since it has been demonstrated that reducing the steady-state plasma LDL-cholesterol concentration produces a proportional decrease in the risk of developing atherosclerosis 2), a major effort is now underway to develop safe and effective lipid lowering agents. In the past, these efforts have been hampered by a lack of knowledge concerning the physiological regulation of the plasma LDL-cholesterol concentration. However, recently there have been several major advances in these areas and it is now possible to precisely determine those factors that have the greatest effect on either increasing or decreasing the plasma LDL-cholesterol concentration.
