This book is the result of an informal association between the editors which extends back to 1974. At the beginning, it was a tenuous alliance between physicians in quite different medical subspecialties-urology and psychiatry. As the alliance was forged, subspecialty rivalries and mistrust were replaced by a common clinical interest in the diagnosis and treatment of erectile problems. We quickly became aware of the high prevalence of such disorders, how poorly prepared we were to make accurate and re sponsible diagnosis and treatment plans, and how complicated an etiol ogical diagnosis could prove to be. A variety of biological and psychological influences bear on sexual function, and in many clinical contexts, diagnosis and treatment planning involves consideration of complex interactive var iables. The need for an expanded multidisciplinary team became obvious. The requisite knowledge base extended across too many subspecialty boundaries, and the necessary information was not available in a conve nient source. As we began accumulating the information base, we became aware that this information might be of value to other physicians. The authors enjoy the luxury of an academic setting in which special expertise can be readily assembled. Such resources are often unavailable to the physician in practice. Impotence. The word strikes at the bedrock of a man's worth. Its implications far exceed the inability to have a penile erection. The man is without power-inef fectual. Ancient statues represented with an oversized erect penis attest to the his toric conscious meaning of the phallus. Magnificent spires, memorial monuments, and the awe generated by the sight of rockets and missiles attest to the unconscious meaning. At the medical-superstitious level, men have invoked countless strategies for improving or restoring potency. They have included ritualistic dances, concoctions, and mechanical aids. Some survive to the present. More recently, verbal therapies have taken their place, ranging from simple exhortation to the uncovering of complex unconscious symbolic conflicts. Other therapies borrow from theories of learning, based on experiments with mice and men. The growth of modern endocrinology, with the isolation of "sex steroids," heralded another treatment strategy: hormones. Later, engineers entered with sur gically implantable mechanical devices. And, most recently, the technology of microsurgery has offered the possibility of physiological restoration of a compro mised vascular supply, a necessary component of erection.

First published in 1975. The last two decades have brought remarkable advances in our knowledge of human sexuality. These data are in the process of being assimilated into the main body of psychiatric thought, which is being greatly enriched thereby. Our increased understanding of sexuality is also currently being translated into innovative new approaches to the treatment of sexual difficulties. These developments promise relief to many persons with distressing sexual problems who were previously thought to be beyond help. At the present time, the specific approach to sex therapy described in this volume is being employed, further developed and, most important, systematically evaluated at the Sex Therapy and Education Program of the Payne Whitney Psychiatric Clinic of the Cornell University-New York Hospital Center. The Cornell program is psychiatrically oriented. It regards sex therapy as a specialized branch of psychotherapy. The Clinic conceives of sexual dysfunctions as psychosomatic symptoms and it’s orientation is multicausal and eclectic in that it believes that sexual dysfunctions are the product of multiple etiologic factors, and our treatment armamentarium comprises an amalgam of experiential, behavioral and dynamically oriented modalities.
