According to Goodhart, about half of those who suffer from lesions producing an obstacle to the emission of urine die of pyelonephritis. Post mortem this lesion is found in 41 per cent. Of patients suffering from urethral stricture, and in 74 per cent. Of those who suffer from hypertrophy of the prostate. Excerpt from Diseases of the Genito-Urinary Organs and Kidney It has been the purpose of the writers to present in this volume a discussion of the more important disease conditions of the uro-genital tract, taken from the standpoint of the general practitioner and surgeon. In so far as possible they have attempted to incorporate such methods as they personally have found most practical and useful, all of which they believe may be successfully employed in the hands of any well equipped practitioner, familiar with modern medical and surgical technic. The writers do not profess that the book is complete; this would be impossible in a work of this size. They have attempted to devote the greatest amount of space and the fullest descriptions to those conditions and methods which have appeared to them to be of the greatest importance, or to those which, being of recent development, may be presumed to be less familiar to the practitioner. A larger amount of space has been devoted to the urinary organs proper, and relatively less has been said of purely sexual disorders. The work is the conjoint product of a surgeon and a physician, and it is intended that equal attention should be devoted to both medical and surgical aspects of these diseases. References to literature have not been exhaustively made. Casper's two latest experiments are not more decisive; the fact that ferrocyanide of potassium is found in the contents of the bladder after it has been introduced into the posterior portion of the urethra, and that its existence can be proved with the well-known reaction with iron, is by no means conclusive; pressure caused by instillation could easily force a small quantity of the solution into the bladder, capable of being discovered, thanks to the excessive sensibility of the reaction with ferric ferrocyanide. Finally, the presence of gonococci in the posterior part of the urethra cannot be considered a " proof" in favour of this theory. We quite recognise the difficulties of determining the exact localization of gonorrhoea in the urethra, even with the aid of the endoscope, and we are convinced that the views we have just discussed can no more be relied upon than those put forward in favour of Guyon's theory. We agree, however, with Leprivost, Zeissl, Mracek, and others, that posterior urethritis does not lead to clouding of the urine contained in the bladder, and that this can only take place when paralysis of the sphincter of the bladder is present, so that if the second portion of urine contains pus, it must be considered to be of cystitic origin, a fact which Guyon opposes less than one would expect, after what has been said by his followers. Lesser, also, now admits that in posterior urethritis the second portion of urine may be clear, a phenomenon which he explains by the fact that the pus has " not yet" entered the bladder. Excerpt: ... nephritis is an acute, chronic or recent one, or rather a primary one, or whether it is a fresh outbreak of chronic nephritis; in other cases the question is only solved by examination after the haemorrhagie symptoms have disappeared; sometimes we have not been able to obtain a decided opinion; we lay, however, in opposition to Rosenstein, a great importance on the following signs: hypertrophy of the heart and thread-like pulse; both symptoms are strange to acute and sub-acute nephritis. Bright' retinitis is naturally reckoned among the first symptoms to be looked for. Intense haemorrhagic nephritis, which lasts for months and yields to no treatment, is generally of a chronic origin. On the other hand, fluctuating quantities of blood and casts must not confuse us in the diagnosis of acute, i.e., curable renal inflammation, even when they are noticeable in a moderate quantity for months. Prognosis.--Apart from the cases in which the general malady is of itself specially dangerous (diphtheria, typhus, pneumonia, urticaria, septicaemia, puerperal fever, poisoning, etc.) the secondary kinds offer an exceedingly favourable prognosis, with exception of the most frequent and most important kind, scarlet fever nephritis. Nearly half of those who have nearly recovered become victims, and it is difficult to understand why some writers reckon scarlet fever nephritis to the favourable kinds. The special symptoms so rightly dreaded ars: anuria, lasting sometimes for days together, excessive serous dropsy, especially severe uraemia and extensive inflammation of the internal organs. In most fatal cases noticed by us the urine was only slightly or not at all...
