Les objectifs de la mise en place de cet outil sont : - apporter un télésuivi et une téléexpertise de qualité du patient depuis son lieu de vie, en liaison avec une équipe compétente en plaies et cicatrisation, et en collaboration avec l'équipe hospitalière spécialisée référente et le médecin traitant ; - améliorer la communication entre professionnels, avec une meilleure intégration du médecin traitant au sein de la prise en charge du patient ; - diffuser de bonnes pratiques de prise en charge des plaies complexes, avec, à la clé, une amélioration des compétences de chaque intervenant ; - et enfin se conformer à la législation en cours sur la transmission sécurisée des données médicales et le respect du secret professionnel. L'outil pourra évoluer vers le monde libéral, avec un accès direct pour les médecins généralistes à un spécialiste (en dermatologie, ou autres spécialités), pour une téléexpertise de dossier et un retour rapide de l'information, tout en évitant au patient fragile un déplacement hors de son lieu de vie.

INTRODUCTION : Les plaies chroniques sont un problème majeur de santé publique. Elles ont un retentissement socio-économique lourd. En ville, le médecin généraliste et l'infirmier sont au coeur de cette prise en charge. Notre étude vise à révéler les difficultés rencontrées par ces deux intervenants. METHODE : Étude prospective qualitative et quantitative menée de novembre 2012 à avril 2013 au sein de la consultation plaie et cicatrisation de l'hôpital Rothschild. Des données ont été collectées auprès de 100 patients de plus de 65 ans consultant pour la première fois pour plaie chronique. Les médecins adressant ces patients ainsi que les infirmiers intervenant au domicile ont été interrogés via un questionnaire semi-directif. RESULTATS : 61,6% des médecins et 69,8% des infirmiers ont répondu. La fréquence des difficultés perçues est pour 55% des médecins entre «toujours et souvent » et pour 92% des infirmiers « parfois ». Les difficultés des médecins sont multiples: pour 37%, il s'agit de la difficulté thérapeutique, pour 18% de l'insuffisance de formation et pour 12% de la difficulté diagnostique. L'IPS est sous-utilisé, dans 66% des cas il n'est pas transmis au spécialiste. La durée de la consultation et la réfection du pansement constituent respectivement pour 12% et 15% des médecins un obstacle à la bonne prise en charge. CONCLUSION : L'insuffisance de formation, générant des difficultés diagnostiques et thérapeutiques, représente un obstacle majeur dans la prise en charge. Par ailleurs, la collaboration avec les infirmiers doit être améliorée. Enfin, un meilleur rapport temps de consultation/coût optimiserait les soins, au même titre que l'usage de la télémédecine.
