Malignant mesothelioma is an enormous global health problem related to asbestos exposure. Despite the best efforts of scientists and oncologists, the prognosis for those affected remains poor. Due to anatomical characteristics and non-specific symptoms, the diagnosis of mesothelioma at an early stage is often difficult, while surgery and radiotherapy are only of limited use, even if some multimodality approaches seem promising. In turn, medical treatments are sometimes successful in tumor control, but have little impact on overall survival. However, advances in our understanding of the disease’s biology, together with the availability of new drugs and combinations, make mesothelioma an essential and highly topical field for pre-clinical and clinical studies. This book is subdivided into four parts: epidemiology and preclinical data, diagnosis, therapy, and extrathoracic mesothelioma. It highlights the progress made in a variety of areas – e.g. in vitro and in vivo experimental models, genetics, environment, biomarkers, targeting agents, immunotherapy, metabolic imaging and ongoing clinical trials – and describes the standard clinical management of mesothelioma patients, including those with extra-thoracic localizations. Recommendations Because of the lack of sufficient high-quality evidence on the surgical management of mesothelioma, the Lung Cancer Disease Site Group opinion is that: • The role of surgery in the management of malignant pleural mesothelioma cannot be precisely defined. [...] Specifically, the lack of randomised controlled clinical trials makes it impossible to determine whether the use of extrapleural pneumonectomy or pleurectomy improves the survival of patients with malignant pleural mesothelioma or effectively palliates the symptoms of the disease. [...] Pleural fluid control improved in 98% of 50 patients and 96% of 54 patients; the recurrence of pleural effusion was prevented in 80% of 20 patients; dyspnea improved in 47% of 20 patients and 100% of 37 patients; and pain improved in 21% of 19 patients and 85% of 71 patients. [...] The former procedure generally involves the excision of sections of the pleura, and the latter, more aggressive approach involves the removal of all or part of a lung as well as the parietal pleura and ipsilateral pericardium and diaphragm. [...] Synthesizing the Evidence A statistical synthesis of the evidence was not conducted because no randomized trials involving surgical treatment for mesothelioma were identified and the prospective and retrospective studies included a variety of adjuvant treatments.

Malignant pleural mesothelioma (MPM) is an aggressive malignancy with a poor prognosis, principally caused by a prior asbestos exposure. Up to the present, multimodality protocols including surgery with chemotherapy (CT) and/or radiotherapy (RT) represent the therapeutic gold standard for selected patients (epithelial and early-stage MPM). In this context, the aim of surgery is to accomplish the macroscopic complete resection (MCR). There are two main surgical options to obtain MCR,Äîextrapleural pneumonectomy (EPP) and pleurectomy/decortication (PD). The superiority of one surgical approach over the other is still discussed. To date, the decision to carry out one or the other in a multimodal setting is established on surgeons,Äô preference more than on strong scientific evidence. Due to the high morbidity, both surgical techniques should be achieved in tertiary referral centres. In summary, surgery, CT, and RT have failed as single modality therapies with no effects on patients survival. This aspect may be justified by the lack of randomized trials. Thus, novel therapeutic strategies, such as multimodality treatment and targeted agents, seem to prolong the survival and the quality of life. The aim of this chapter is to provide a complete overview of the current surgical approaches to MPM, discussing within the frameworks of pre-operative diagnostic evaluation and multimodality oncological treatments.
