Le plan national pour le développement des soins palliatifs et l'accompagnement en fin de vie 2015-2018 présentait comme l'un de ses objectifs le développement des soins palliatifs au domicile. Que ce soit en libéral, en Hospitalisation à Domicile, en Service de Soins Infirmier à Domicile, ou en réseaux de soins palliatifs, les infirmiers étaient au coeur des soins du patient en fin de vie. L'objectif de cette étude était de comprendre les enjeux de la prise en charge de la fin de vie au domicile dans une situation palliative, au travers du regard des infirmiers. Méthode : Il s'agissait d'une étude qualitative. La population étudiée était constituée d'infirmiers diplômés d'état sur le territoire de la Communauté de l'agglomération Havraise. L'entretien comprenait deux parties : la première vérifiait la variabilité de l'échantillon, la deuxième était un questionnaire semi dirigé individuel ou à deux, qui explorait les pathologies, les expériences, les échanges d'informations, les moyens, les difficultés, et les attentes dans la prise en charge de la fin de vie à domicile. Résultats : Onze infirmiers ont été interrogés au cours de neuf entretiens. La saturation de donnée a pu être obtenue au cours du dixième infirmier interrogé. Les patients suivis étaient principalement les personnes âgées et ceux atteins de cancer. Les infirmiers souhaitaient pouvoir suivre leurs patients et l'entourage jusqu'au bout, et étaient prêts à prendre sur leur temps. Le lien avec le médecin traitant était primordial : informations, ordonnance, prises de décisions et annonce. Les équipes d'appui étaient des aides capitales pour la coordination avec les différents professionnels impliqués et pour les conseils techniques pour une meilleur prise en charge. Les principales difficultés étaient le manque de communication entre les professionnels, l'isolement de certains patients, et le manque de ressources financières. Discussion : De-nombreuxxofessionnels gravitaient= autour du patient en fin de vie-: médecin traitant, soignants, spécialistes, équipes d'appui. C'est la bonne coordination de ces derniers qui permettaient d'obtenir une prise en charge optimale c'est-à-dire avec le confort, sous tous ses aspects, du patient, et la prise en charge de son entourage. Cependant les problèmes de communication, à tous les niveaux, pouvaient rendre difficile cette prise en charge. Les réflexions éthiques tentaient d'apporter les réponses aux attentes de la population, mais le sujet divisait les populations.
