Cerebral Sphingolipidoses: A Symposium on Tay-Sach's Disease and Allied Disorders is a collection of papers presented at the 1961 Symposium on the Cerebral Sphingolipidoses, held in Isaac Albert Research Institute of the Jewish Chronic Disease Hospital and the Downstate Medical Center of the State University of New York. The contributors demonstrate the significance attached to an intensive study of Tay-Sach's and allied diseases. A unique feature of the Symposium was the constant awareness of the clinical implications of the fundamental studies presented. This book is organized into three sections encompassing 30 chapters. The first part deals with the morphological aspects of cerebral lipidoses, including the diagnosis, biopsy, pathology, and clinical features of these diseases. The present volume contains the scientific contributions to the Fifth International Symposium on "Current Trends in Sphingo lipidoses and Allied Disorders" under the auspices of the Isaac Albert Research Institute of the Kingsbrook Jewish Medical Center, the Department of Pathology, Downstate Medical Center, State Uni versity of New York, Brooklyn, New York, and the National Tay-Sachs and Allied Diseases Association, Inc., New York. A review of the four previous Symposia shows the increase in scope of the scientific exploration in this rapidly expanding field. The first meeting, held in 1958, was devoted to the discussion al most entirely of Tay-Sachs disease. The majority of the work emanated from local laboratories. The participants at the present Symposium came from many other domestic and foreign research in stitutions. The scope of the papers presented at these meetings and the interest shown in the Symposium demonstrates the signifi cance attached by the scientific community to the problems of these hereditary diseases. The reasons for this are apparent, when one considers the contributions during recent years to our basic know ledge by lipid and enzyme chemistry, genetics, and neuropathology. Partly because of the hereditary nature of these diseases any new discovery in this field has general meaning and permits cautious generalization well beyond its clinical significance.

For the past 30 years I have been teaching lipid biochemistry to'inedical students, graduate students, and undergraduate students. The major topics covered in my courses were fatty acids, prostaglandins, leukotrienes, phospholipids, glyco lipids, triacylglycerols, cholesterol, bile acids, and plasma lipoproteins. Empha sis was placed on the regulation and disorders of lipid metabolism. The latter included hyperlipidemias, atherosclerosis, and alcohol-induced liver damage. In this volume, I have chosen to focus on the disorders of lipid metabolism at a level appropriate both for medical students and for graduate and undergradu ate students majoring in the biological sciences. The biochemistry, nutrition, genetics, and cell biology aspects of lipids and lipid metabolism will be covered as they relate to lipid disorders. I am not aware of any textbook that integrates the disorders of lipid metabolism in this manner. Chapter 1 includes a brief discussion of the basic structures, properties, and metabolism of lipids. This chapter is not very detailed, since the material covered is available in basic textbooks on biochemistry. The major fpcus of this volume is the various lipid disorders, with emphasis on polyunsaturated fatty acids, the molecular biology and pathogenesis of the hyperlipidemias, dietary and drug therapy for the hyperlipidemias, and alcohol-induced liver damage. The material presented has been obtained from several textbooks on biochemistry and from a variety of recent articles in the scientific literature.

I was gratified by the most favorable reception and wide usage received by the first edition of this book. A decade seems to be a short period for a book on pathology, and yet it witnessed many important changes of concepts, along with a formidable growth of knowledge. The second edition required extensive reorganization. There are new chapters on mitochondriopathies, on peroxisomal diseases and on spongy myelino pathies. Major revisions and new additions were necessary in many chapters, for instance those on the dysplasias of the cerebral and of the cerebellar hemispheres, which were largely reorganized. The chapters on perinatal pathology were reordered and reorganized to give a more logical sequence of prenatal, perinatal and postnatal lesions. The entire text was worked over for brevity. A wealth of new references was added with the. aim of staying abreast with the literature up to summer 1988. All refer ences were double checked for errors. My gratitude goes to Mrs. Gisela Ropte and Mrs. Cynthia Bunker for their untiring, diligent help. As a result, this second edition is an essentially rewritten text. Advance in the prevention of human suffering is based on a thorough understand ing of the nature of disease. I hope that this text will continue to be of service in this behalf.
