Introducción: La obesidad es un grave problema de salud. México Ocupa el primer lugar en obesidad infantil, cerca del 40% de los niños padecen sobrepeso u obesidad, este se asocia a trastornos en los carbohidratos, diabetes tipo 2, dislipidemias, hipertensión arterial, esteatosis hepática y la posibilidad de Síndrome Metabólico. Objetivo: Identificar el posible efecto del sobrepeso u obesidad (SP u 0) sobre las alteraciones de los marcadores bioquímicos y hormonales e hígado graso en adolescentes de 12 a 15 años de edad. Material y Métodos: Estudio Transversal comparativo integrado por dos grupos 170 adolescentes con SP u O (grupo1) y 131 con peso normal (grupo 2) N=321, muestreo aleatorio estratificado de tres escuelas de secundaria de Satillo, Coahuila. Se incluyeron variables antropométricas, sociodemográficas, bioquímicas, hormonales y clínicas, así como ultrasonido hepático. Resultados: En cuanto al sexo y edad no hubo diferencias significativas entre ambos grupos. El antecedente de obesidad en alguno de los padres, presentó diferencia significativa (OR 2,67; IC: 1,54-4,63). En cuanto a las variables clínicas, resultaron con diferencia significativa el signo de acantosis nigricans (OR: 7,99; IC: 7,99- 17,46), e Hipertensión arterial diastólica 8,2% (OR: 9,50; EIC: 1,17- 76,94). Al analizar las variables bioquímicas hubo diferencia significativa en hipertrigliceridemia (OR: 7,56; IC: 2,50- 22,87) y HDL (19,28; IC: 2,50-148,17. En relación a las variables hormonales, hubo diferencia significativa en valores altos de insulina (OR: 17,87; IC: 6,73- 47,43) y el índice de HOMA/IR elevado (OR: 9,37; IC: 5,06- 17,35) y la presencia de esteatosis hepática por ultrasonido OR: 19,90 IC: 6,87- 58,07). En el 9% de los adolescentes con Sp u obesidad se integró el Síndrome Metabólico. Conclusiones: Es evidente el efecto nocivo que ejerce la obesidad sobre las alteraciones a nivel de los lípidos, carbohidratos, hipertensión arterial e hígado graso; todos ellos factores de riesgo cardiovascular.
