The medical research landscape in the United States is supported by a variety of organizations that spend billions of dollars in government and private funds each year to seek answers to complex medical and public health problems. The largest government funder is the National Institutes of Health (NIH), followed by the Department of Defense (DoD). Almost half of DoD's medical research funding is administered by the Congressionally Directed Medical Research Programs (CDMRP). The mission of CDMRP is to foster innovative approaches to medical research in response to the needs of its stakeholdersâ€"the U.S. military, their families, the American public, and Congress. CDMRP funds medical research to be performed by other government and nongovernmental organizations, but it does not conduct research itself. The major focus of CDMRP funded research is the improved prevention, diagnosis, and treatment of diseases, injuries, or conditions that affect service members and their families, and the general public. The hallmarks of CDMRP include reviewing applications for research funding using a two-tiered review process, and involving consumers throughout the process. Evaluation of the Congressionally Directed Medical Research Programs Review Process evaluates the CDMRP two-tiered peer review process, its coordination of research priorities with NIH and the Department of Veterans Affairs, and provides recommendations on how the process for reviewing and selecting studies can be improved.

While the principles are fundamentally generic, and apply to peer review across the full spectrum of performing institutions as well as manuscript, proposal, and program peer review, the focus of this report is peer review of proposed and ongoing research programs in federal agencies. In 1993 Congress allocated $210 million for breast cancer research as part of the Department of Defense budget. An Institute of Medicine (IOM) committee was convened at that time to advise the U.S. Army Medical Research and Development Command on strategies for managing a breast cancer research program. This book evaluates the program's management and achievements to date. Although it is too early to evaluate the program in terms of breakthrough results and new insights produced by the funded projects or investigators, this book documents the process used to select research proposals for funding and analyzes the portfolio of funded projects in terms of their responsiveness to the recommendations and fundamental questions articulated in the 1993 IOM report.

In the three decades since the U.S. Environmental Protection Agency (EPA) was created, the agency's scientific and technical practices and credibility have been independently assessed many times in reports from the National Research Council (NRC), EPA Science Advisory Board, General Accounting Office, and many other organizations; in congressional oversight and judicial proceedings; and in countless criticisms and lawsuits from stakeholders with interests in particular EPA regulatory decisions. As a previous independent panel put it in the 1992 report Safeguarding the Future: Credible Science, Credible Decisions, EPA's policy and regulatory work receives a great deal of public attention, but the agency's scientific performance typically receives a similar degree of attention only when the scientific basis for a decision is questioned. Thus, strong scientific performance is important not only to enable EPA to make informed and effective decisions, but also to gain credibility and public support for the environmental protection efforts of EPA and the nation. This report is the fourth and final one in a series prepared by two independent expert committees convened by the NRC in response to a request from Congress and to subsequent, related requests from EPA. The Committee on Research Opportunities and Priorities for EPA - the companion committee in this study - was charged to provide an overview of significant emerging environmental issues, identify and prioritize research themes most relevant to understanding and resolving those issues, and consider the role of EPA's research program in the context of research being conducted or supported by other organizations. That committee published an interim report in 1996 and a final report, Building a Foundation for Sound Environmental Decisions, in 1997. The Committee on Research and Peer Review in EPA was charged to evaluate research management and scientific peer-review practices in the agency. The committee published an interim report in 1995 and this final report.

The Office of Science and Technology (OST) of the U.S. Department of Energy's (DOE's) Office of Environmental Management (EM) recently has instituted a peer review program that uses the American Society of Mechanical Engineers (ASME), with administrative and technical support provided by the Institute for Regulatory Science (RSI), to conduct peer reviews of technologies (or groups of technologies) at various stages of development. OST asked the NRC to convene an expert committee to evaluate the effectiveness of its new peer review program and to make specific recommendations to improve the program, if appropriate. This is the first of two reports to be prepared by this committee on OST's new peer review program. OST requested this interim report to provide a preliminary assessment of OST's new peer review program. In the final report, the committee will provide a more detailed assessment of OST's peer review program after its first complete annual cycle.
