Le suivi gynécologique des femmes est un véritable enjeu de santé publique. La tendance démographique actuelle est celle du déclin des gynécologues médicaux. Le médecin généraliste devrait être confronté à une augmentation de son activité gynécologique. Cette étude qualitative par entretiens semi-dirigés auprès de neuf médecins généralistes Rhônalpins a pour objectif de comprendre les freins et les moteurs de leur pratique gynécologique. Pour la plupart des médecins interviewés, la gynécologie représente une part non négligeable de leur activité. Cette dernière porte essentiellement sur le dépistage des cancers, la prescription et surveillance de pilule et le suivi de grossesse. La majorité d'entre eux a exprimé son intérêt pour la gynécologie. Plusieurs moteurs de la pratique gynécologique ont été exprimés. Il s'agit avant tout de la demande émanant des patientes, liée à l'indisponibilité croissante des gynécologues. Le sexe féminin des praticiens et la relation de confiance établie entre une patiente et son médecin traitant sont aussi facilitants. L'intérêt pour la discipline ainsi qu'une bonne formation sont une motivation supplémentaire, d'autant que la plupart des médecins interviewés considèrent que la gynécologie fait partie de leurs compétences. Les difficultés sont liées à un manque de compétence ressenti et/ou un manque de formation, notamment sur le plan technique; à un manque de temps, pour pratiquer et pour se former; à la réticence des patientes à les consulter (compétences jugées insuffisantes ou non connues, pudeur due à l'ancienneté de la relation médecin-patiente, sexe masculin du praticien). Pour pallier la diminution de l'offre de soins, les médecins généralistes vont devoir développer un partenariat avec les professionnels impliqués dans le suivi de la femme: gynécologues médicaux et obstétricaux, sages-femmes libérales et médecins des CPEF. Les lieux de réalisation des FCU pourraient être diversifiés et généralisés à l'ensemble des laboratoires d'analyses médicales. Et une meilleure diffusion auprès de la patientèle des compétences du médecin généraliste à pratiquer la gynécologie, à prendre en charge les problèmes féminins les plus prévalents, s'avère nécessaire

Contexte : Le suivi de grossesse à bas risque peut être effectué par un gynécologue, une sage-femme ou un médecin généraliste. Malgré la pénurie de gynécologues, peu de suivis de grossesse sont réalisés par les médecins généralistes. Objectifs : L'objectif principal de cette étude était de connaitre les raisons pour lesquelles certaines patientes décidaient de confier le suivi de leur grossesse à leur médecin généraliste. L'objectif secondaire était d'identifier les attentes des femmes pour ce suivi de grossesse. Méthode : Enquête qualitative descriptive par entretiens semi-dirigés auprès de patientes en suites de couches, ayant été suivies par un médecin généraliste jusqu'au début du 6ème mois de grossesse. Enregistrement des entretiens, retranscription intégrale et analyse thématique du contenu. Résultats : Les critères principaux de choix du médecin généraliste comme professionnel du suivi de grossesse étaient de deux ordres. D'un point de vue organisationnel, tout semblait « plus simple » : souvent plus accessible en termes de proximité géographique et de disponibilité, globalité du suivi, simplification des démarches... D'un point de vue humain, il semblait exister une relation particulière avec leur médecin, avec une connaissance mutuelle rassurante, gage de confiance. Les patientes reconnaissaient sa réactivité et ses compétences. Conclusion : Rares sont les grossesses suivies en cabinet de médecine générale. Pourtant, le médecin généraliste semblait répondre aux attentes relationnelles et organisationnelles de certaines patientes pour qui relation et proximité seraient plus importantes que la technicité proposée par le gynécologue.

Le taux de consultation spontanées aux urgences reste à ce jour encore très élevé, même aux urgences gynéco-obstétriques. L'objectif principal de cette étude est de connaître les motivations des patientes qui consultent spontanément aux urgences gynécologiques. L'objectif secondaire est de comparer le devenir et la prise en charge des patientes selon qu'elles soient ou non adressées par un professionnel de santé aux urgences gynécologiques. Pour cela nous avons réalisé une étude descriptive à partir de la distribution de questionnaires aux patientes consultant aux urgences gynécologiques du CHU de Nantes entre janvier et juillet 2020. Au total 80 questionnaires ont été récupérés et analysés. Les patientes ont été réparties en 2 groupes : le groupe 1 concernant les patientes consultant spontanément aux urgences gynécologiques et le groupe 2 concernant les patientes qui avaient eu une consultation ou un avis médical préalable avant leur arrivée aux urgences. Dans le groupe 1 les motivations principales des patientes étaient le sentiment d'avoir besoin d'examens complémentaires et la situation d'urgence ressentie. De nombreuses patientes déclaraient également penser que leur médecin traitant n'était pas apte à les prendre en charge, pour les symptômes qui les faisaient consulter aux urgences. Entre les 2 groupes le devenir après la consultation et les examens complémentaires réalisés n'étaient pas significativement différents. Le nombre de passage aux urgences gynécologiques est en constante augmentation en France. Le développement d'un réseau de ville par les médecins généralistes afin d'orienter les patientes vers les examens appropriés, comme l'échographie pelvienne, apparait comme essentiel. Enfin il apparaît important d'informer les patientes sur les possibilités ou non de suivi gynécologique par les médecins généralistes.
