La question de la fin de vie est un enjeu sociétal majeur. Le développement des soins palliatifs a permis d'améliorer la prise en charge de cette période, notamment par la création d'équipe mobile de soins palliatifs. Ces équipes initialement intra-hospitalières, ont vu leur champ d'action s'élargir au domicile du patient et au sein des structures médicosociales. Il a été montré que leur intervention permettait de limiter l'agressivité des soins en fin de vie. Pour définir le profil de personnes âgées décédant aux urgences, nous avons réalisé une étude épidémiologique, observationnelle, rétrospective et descriptive des décès de patient de plus de 65 ans, adressés par des établissements pour personnes âgées de la région lyonnaise aux urgences de l'hôpital Saint Joseph Saint Luc. Nous avons inclus les services du SAU, de l'UHCD et de l'UHP. Nous avons recensé 317 décès de patients âgés de plus de 65 ans au sein des services étudiés. Parmi eux, 140 provenait d'un établissement pour personnes âgées (USLD, EHPAD, EHPA non EHPAD, résidences autonomie). Les femmes représentaient 66% de notre population. Les patients présentaient un niveau de dépendance GIR 1 et 2 pour 49% d'entre eux. Ils étaient adressés en majorité pour des difficultés respiratoires. Le décès est survenu en moyenne 2,5 jours après l'admission aux urgences. Les principaux traitements administrés étaient à visé antalgique, respiratoire et anti-infectieuse. Une décision collégiale de limitation des soins a eu lieu pour 61 patients. La quasi-totalité des patients n'a pas bénéficié de l'intervention d'une EMSP. La diffusion d'outils d'aide à la prise en charge comme les fiches SAMU PALLIA et PALLIA 10 GERONTO permettrait de favoriser le développement des soins palliatifs au sein des établissements médico-sociaux.
