This book is designed to meet the need for a practically oriented textbook on the rapidly growing field of continuous EEG (cEEG) monitoring. A wide range of key clinical aspects are addressed, with explanation of status epilepticus classification, criteria for institution of monitoring, seizure patterns and their recognition, quantitative EEG analysis, and neuroimaging in patients undergoing cEEG monitoring. The value of cEEG and the nature of cEEG findings in various special situations are then reviewed, covering particular pathologies, critical care considerations, and prognostication. Treatments of nonconvulsive status epilepticus (NCSE) and nonconvulsive seizures (NCS) are discussed. The concluding section is devoted to important administrative issues including billing, staffing issues, comparison of EEG machines, and information technology (IT) issues.Continuous EEG monitoring offers the only reliable means of detecting seizures that are not clinically obvious in critically ill patients. Analysis of EEG records The book begins by summarizing the physiology and the fundamental ideas behind the measurement, analysis and modeling of the epileptic brain. It introduces the EEG as a measured signal and explains its use in the study of epilepsy. Next, it provides an explanation of the type of brain activity likely to register in EEG measurements, offering quantitative analysis of the populations of neurons that contribute to both scalp and cortical EEG and discussing the limitations and effects that choices made in the recording process have on the data. For patients with medically refractive focal epilepsy, surgical intervention to remove the epileptic foci is often the last alternative for permanent cure. The success of such surgery is highly dependent on the doctor's ability to accurately locate the epileptogenic region during the pre-surgical planning and evaluation phase. Hence the goal of this project is to provide an end-to-end quantitative analysis pipeline that fuses an array of imaging modalities including magnetic resonance imaging (MRI), diffusion tensor MRI, positron emission tomography (PET), single-photon emission computerized tomography (SPECT) as well as EEG data to build patient-specific head models and to compute prior probability maps of epileptic hotspots for more accurate EEG source localization. By improving the ability to accurately locate these epileptogenic seizure sources, patients can benefit tremendously from accurate surgical resection and consequently have a better chance for complete seizure free recovery.
