The peri-menopause is emerging more and more as a subject for study by the doctor, the psychologist, the social worker, the gerontologist, the clinical chemist, the public health specialist, the sociologist, the psychiatrist and the experimental pathologist. The results of studies are gradually becoming known to the public, particularly to the prospective clients, ageing women. Women have begun to ask for treatment and they will most likely determine whether oestrogen replacement therapy comes into vogue. Psychosomatics in peri-menopause is an important sub ject, since the life-span of the woman has extended to nearly twice that of her fertile period. The life-expectancy for women in industralized countries is now some 70-75 years. This means that most can now expect to spend one third of their lives in the post-menopause. Consequently, there is now much more time than before in which to notice, experience and be bothered by peri menopausal changes. Moreover modern medicine has much more to oner than mere moral support to sufferers from ailments connected with progressing ovarian failure during peri-menopause. Although we cannot promise eternal youth - as Wilson does -there are more possibilities for treatment than 10 years ago.

Volume 3 of this series is concerned with geriatric aspects of surgical specialties: gynecology, orthopedics, general surgery, otorhinolaryn gology, and ophthalmology. Closely associated with these specialties is anesthesiology. Dermatology has an intermediate position between surgical and nonsurgical fields. The peculiarities of physiological and pathological aging of otgans and the consequences for diagnosis and therapy - presented in the first two volumes - are of great significance, especially for surgical special ties. There are a large number of pre-, intra-, and postoperative problems in multimorbid geriatric patients, e. g. , coronary insufficiency, brady arrhythmias, hypertonia, and hypotonia. While as recently as the tum of the century the age of 65 years was viewed as a contraindication for sur gery, today even older patients undergo operations on aortic aneurysms, bypass operations for coronary sclerosis, pulmonary resections, and abdominosacral resections of rectal carcinomas, for example. Pre requisite for successful surgery at an advanced age is good pre- and postoperative care of multimorbid patients. Physiological changes of the lungs with aging, the increased frequency in pneumonia and pulmonary just a few embolisms with age, and the decrease in receptors, to give examples, confront anesthetists with difficulties. The maxim "in old age a little less" is also applicable in this field. Only improved experimental gerontological research, possibly reaching even into anesthesia, will provide objective data for anesthesia in elderly patients. The skin is an organ that experiences characteristic qualitative and quantitative changes in old age.

We do not think about everything at once all the time. Various mecha nisms allow us to choose from among the themes, issues, topics, feelings, ideas, and memories that might occupy consciousness. One can focus selectively on anything deemed important; yet the methods by which this is accomplished vary greatly. We clinicians assign to these various mech anisms names that fit whatever theoretical system is central to our work-the healthy suppression of "background noise" allows us to pay attention to certain matters; the repression of unconscious conflict may assist our functioning in one moment despite its later cost; whereas denial and disavowal are used as general and fairly nonspecific terms for matters that are left out of awareness in order to avoid the noxious emotions specific to the personal significance of such awareness. Despite the attitude of scientific objectivity characterizing Freud's introduction of psychoanalysis, an aura of morality clings to certain of these mecha nisms, for we tend to judge people by their use of them. We are a society of doers, people of action and accomplishment who look with disrespect at the avoidance of any responsibility or task. Thus denial has taken on a negative connotation, and those who use this avoidance system are seen as the lesser among us.

Three International Congresses on the Menopause have been held during the last five years, evidence of the explosive increase in the scient ific study of this subject. The first, held in La Grande Motte, near Montpellier, France, inJune 1976, was designed to provide a consensus on menopause research (van Keep et al., 1976). The second, held in Jerusalem, was planned to assess the developing research (van Keep et al., 1979). The objective of the third congress, convened inJune 1981 in Ostend, Belgium, under the auspices of the International Menopause Society, was to explore areas of controversy in basic understanding and in the therapeutic advances relating to the climacteric. The workshop moderators were selected because of their acknow ledged leadership in menopause research, and each was invited to arrange a workshop on a specific area of interest or concern. The moderators, whilst being given a free hand as far as the selection of invited speakers was concerned, were asked to allow plenty of time for open discussion. In particular, they were encouraged to stimulate debate about controversial issues.
