Les composites ont suivi de très nombreuses améliorations au cours de ces dernières années. Les défauts majeurs ont été largement corrigés : l’usure est devenu proche de celle de l’émail, la rétraction de prise est ramenée à 1,5%, les propriétés mécaniques sont suffisantes. L’objectif de toutes restaurations est de reconstituer les tissus perdus en reproduisant les propriétés optiques par l’utilisation d’une technique de stratification multicouches. L’intégration esthétique passe donc par l’utilisation de biomatériaux qui se comportent, lors de l’exposition à la lumière, comme la dent naturelle. Celle-ci est composée de deux tissus distincts, l’émail, et la dentine caractérisés par une structure histologique bien spécifique à l’origine des difficultés de reproduction de celle-ci. Le succès de leur remplacement sera le fruit d’une stratification anatomique afin de recréer la translucidité, la transparence et les effets de fluorescence et d’opalescence. La restitution de l’architecture interne et externe permet à la lumière de circuler dans la restauration comme dans la dent naturelle, condition unique à l’intégration esthétique. Cependant à l’heure actuelle la littérature proposant et définissant la technique de stratification limite son utilisation à la restauration des fractures et des lésions cervicales d’usure. Or l’optimisation des résines composites grâce aux nanotechnologies ne nous permet-elle pas de généraliser ou au moins accroitre ses indications ?
