Les recommandations préconisent l'emploi d'antalgiques de paliers 3 dans leur prise en charge. Mais elles apparaissent comme sous traitées en médecine de ville et le recours aux morphiniques faibles. De même, leur prise en charge en territoire isolé se révèle complexe du fait de contraintes géographiques, l'éloignement des structures d'urgences pouvant être supérieur à 30 minutes. Le médecin Correspondant du SAMU est un médecin généraliste volontaire, exerçant à plus de 30 minutes d'accès d'une structure d'urgence, formé à l'urgence et bénéficiant d'une large dotation médicamenteuse. Il s'agit d'un effecteur d'aide médicale urgente de proximité. Dans ce travail, nous avons voulu évaluer qualitativement l'apport du système MCS dans la prise en charge de la douleur aiguë intense (DAI), par entretiens semi-dirigés réalisés auprès de MCS. L'analyse thématique des entretiens révèle que ceux-ci utilisaient déjà systématiquement de la morphine intraveineuse pour les DAI avant leur adhésion au système MCS. Ce système a optimisé la prise en charge de celles-ci par la formation dispensée, la dotation médicamenteuse et en matériel de surveillance, la protocolisation et la coordination avec le SAMU. Cette optimisation apparait pour ces médecins comme un bénéfice pour le patient, pensent mieux les prendre en charge et estiment ne pas voir plus d'effets indésirables liés à l'antalgie, grâce au matériel de surveillance et l'oxygène.
