Introduction : Les convulsions sont fréquentes en pédiatrie et font l'objet d'une prise en charge encore trop variable. L'objectif est de décrire les crises convulsives aux urgences pédiatriques à La Réunion, et d'essayer de participer à l'amélioration de leur prise en charge. Méthode : Dans cette étude prospective menée d'Octobre 2011 à Avril 2012 dans deux hôpitaux de la Réunion (St Denis et St Paul), ont été inclus tous les enfants de moins de 18 ans se présentant aux urgences pédiatriques pour une crise convulsive. Nous avons recueilli les caractéristiques cliniques des crises, leur prise en charge et le devenir de ces enfants dans les 6 mois. Résultats : Nous avons observé 294 convulsions, survenues chez 250 enfants. Cela représentait 3,5% des consultations à St Denis. La plupart de ces enfants avaient moins de 5 ans (68%) et le sex ratio était de 1,5. La grande majorité des crises (71,5%) étaient généralisées tonico-cloniques et assez peu d'EME ont été observés (5%). Une épilepsie était la cause de la crise dans 32% des cas. Nous avons décrit une majorité de CF, simples le plus souvent (69,1%), recurrentes dans 11,7% des cas. Nous avons compté 67% de biologie sanguine réalisée, et de façon sans doute excessive (48% pour CFS), et 14,3% d'imagerie, dont deux pathologiques. La moitié des enfants a été hospitalisé. Deux décès sont survenus dont l'un en rapport avec une méningite à pneumocoque. 70% des enfants suivis se portaient bien sans récidive dans les 6 mois. Conclusion : Les convulsions sont un motif fréquent de recours aux urgences pédiatriques. Les convulsions fébriles simples sont les plus fréquentes. Ces convulsions sont la plupart du temps bénignes mais l'infection du SNC, qui est grave, doit toujours être évoquée. La prescription d'examens complémentaires et l'hospitalisation restent fréquentes et variables d'un hôpital à l'autre. Nous proposons un aide mémoire pour la prise en charge de ces convulsions de l'enfant.
