Objectifs : décrire et analyser l'épidémiologie des barotraumatismes de l'oreille moyenne et des sinus chez les plongeurs militaires au sein de l'École de Plongée de Saint-Mandrier et du Centre National d'Instruction Nautique de la Gendarmerie afin de déterminer des facteurs de risque de barotraumatismes et mesurer leur impact en termes d'aptitude. Patients et méthode : 381 profils de plongeurs militaires qui ont réalisé une plongée entre janvier 2019 et juillet 2020 au sein de l'École de Plongée de Saint-Mandrier (83) et du Centre National d'Instruction Nautique de la Gendarmerie d'Antibes (06) ont été inclus dans l'étude. Les antécédents des plongeurs et leurs niveaux de plongée ont été recueillis. Lors de la survenue d'un BTOM ou BTS, les paramètres de plongée, le traitement et la durée d'inaptitude ont été recueillis. Résultats : 44 plongeurs ont présenté un BTOM dont 2 avec récidive et 16 un BTS dont 4 avec récidive. Un antécédent d'otites à répétition est un facteur de risque significatif de BTOM (p=0,002), un terrain de rhino sinusite chronique s'exprimant par une obstruction nasale chronique et une rhinorrhée chronique est un facteur de risque de BTS (p=0,009 et p=0,014). Conclusion : 11,5% des plongeurs ont présenté un BTOM et 4,2% un BTS. 24% des BTOM ont causé une inaptitude entrainant la fin de la formation alors qu'aucun BTS n'a entrainé d'échec à la formation. La recherche d'un terrain d'otites à répétition, d'antécédent de BTOM et d'une rhino sinusite associant rhinorrhée et obstruction nasale permettrait une meilleure sélection des plongeurs. Underwater Physiology is a collection of papers that deals with the physiologically limiting effects of undersea, high pressure exposure ranging from fundamental biological reactions, through integration of physiological stresses, and to limits actually experienced in deep diving. Papers discuss oxygen, the mechanisms of toxicity, and the effects of oxygen on cells and systems such as its pathological and physiological influences in the neurosensory ocular tissue. Other papers discuss the physical effects of pressure and gases on cellular function, protein structure, and the possibility of alleviating symptoms through the administration of drugs. Tests in mice show that various gases exhibit qualitative and semi-quantitative differences in the characteristics of sickness, reactions to hypoxia, and the time before the onset of symptoms. A computer, programmed for nonlinear gas transfer and other variables, running in real time can compute directly from the breathing mixture and provide a real time solution to decompression sickness under various conditions. A combined therapeutic approach, recompression and dextran (an effective lipemic clearing agent) should be capable of treating decompression sickness in humans. Other papers investigate the influence of inert gases and pressure on the central nervous system, as well as, situations in undersea and manned chamber operations.
