Introduction : Les médecines complémentaires et alternatives (MCA) séduisent de nombreux patients, et certains médecins font le choix de les pratiquer ou de les prescrire malgré le faible niveau de preuve de leurs bénéfices. Nous avons cherché à décrire les habitudes de prescription, les connaissances et le niveau d'adhésion des médecins généralistes concernant ces thérapies et à dresser le profil des médecins qui les prescrivent. Le propos a été centré sur l'homéopathie, l'ostéopathie, l'acupuncture et la phytothérapie. Matériel et méthode : Le travail s'est construit en une étude épidémiologique observationnelle descriptive et une étude épidémiologique observationnelle analytique. La population était l'ensemble des médecins généralistes du département de la Loire. Le recueil s'est effectué sous la forme d'un questionnaire numérique anonyme envoyé aux médecins par messagerie électronique. Résultats : 136 réponses ont été collectées, provenant de 61 hommes et 75 femmes. Le niveau de connaissance en termes de littérature scientifique concernant ces thérapies était globalement mauvais. Les médecins en étaient majoritairement partisans excepté pour l'homéopathie pour laquelle ils étaient majoritairement réticents. Ils étaient 21.3% à posséder au moins un diplôme ou une formation en médecine alternative. 12.5% n'en prescrivaient jamais, 64.7% n'en prescrivaient que rarement, 19.9% en prescrivaient souvent, et 2.9% en prescrivaient quasi-exclusivement. 31 médecins ont été classés « prescripteurs de MCA » car ils y avaient recours souvent ou quasi-exclusivement. Ils se différenciaient significativement des autres par le sexe féminin, l'âge compris entre 35 et 55 ans, leurs consultations plus longues et leur plus grande préoccupation concernant leur hygiène de vie. Les raisons qu'ils mettaient en avant étaient principalement la conviction de l'efficacité de ces thérapies et leur expérience personnelle. Pour chaque MCA étudiée, il n'a pas été mis en évidence de lien entre le niveau de connaissance et le niveau d'adhésion. Conclusion : Si les MCA ont plutôt bonne image auprès des médecins, la majorité de leurs prescriptions se font dans la méconnaissance de leur niveau de preuve scientifique et bien souvent dans le simple but d'utiliser l'effet placebo. Peut-être qu'une information concernant ces pratiques devrait être proposée dans le cursus médical.

Contextualisation : l’offre et la demande de médecines complémentaires et alternatives (MCA) sont diversifiées et présentes dans le parcours de soin en médecine générale. En France, la médecine conventionnelle et les MCA s’opposent dans un débat de légitimité axé sur l’efficience des MCA, qui se cristallise actuellement autour de l’homéopathie. Malgré le doute sur leur efficience et leurs quasi-absence dans la formation médicale continue, des médecins généralistes pratiquent des MCA. Le modèle de la médecine intégrative les associe dans une approche scientifique validée, centrée sur le patient avec une vision pluridisciplinaire. Objectifs : l’objectif de l’étude était de comprendre les motivations, l’organisation et le vécu des médecins généralistes intégrant une activité MCA dans leur exercice. Méthode : une étude qualitative inspirée de la méthode de la théorisation ancrée a été réalisée par entretiens semidirigés de mai à juillet 2019. Une analyse thématique et un codage sélectif ont permis l’émergence des thématiques transversales. Résultats : Dans les consultations, les MCA prennent trois formes : des prescriptions (homéopathie, aromathérapie, phytothérapie, compléments alimentaires, conseils nutritionnels), des pratiques à médiation corporelle (l’acupuncture, l’ostéopathie) et des approches impliquant l’utilisation de l’esprit (l’hypnose, la méditation, la psychothérapie). Trois profils de médecin ont émergé. L’exercice intégrant une MCA peut être sur un mode complété (MCA en second plan de l’approche conventionnelle), alterné (alternance de consultations conventionnelles et de consultations dédiées aux MCA) et intégré (considération équivalente des différentes approches dans une consultation unique). Discussion : la dissonance cognitive est présente dans le discours des médecins généralistes. L’ambivalence des praticiens s’atténue selon le degré d’intégration des MCA. L’efficacité expérientielle (légitimité pragmatique) et le titre de docteur (légitimité académique) confortent le praticien dans son exercice et vis-à-vis des agents de contrôle de la profession (pairs, communauté scientifique, Ordre, législation). Conclusion : L’exercice médical intégratif est basé sur l’empirisme. Les médecins généralistes adaptent l’EBM(evidence based medicine) selon leurs expériences et leurs préférences/valeurs. Leur exercice est davantage un evidence based practice modulé par les valeurs et préférences individuelles (values based medicine). Des méthodes d’évaluation mixtes et pragmatiques pourraient évaluer l’efficience des MCA dans l’offre de soins.
