Le cancer du sein est le cancer le plus fréquent en France avec une incidence qui ne cesse d’augmenter. Le taux de participation minimum au dépistage organisé espéré pour permettre une réduction significative de la mortalité liée au cancer est de 70%. Dès son expérimentation, ce DO s’est heurté à la difficulté d’adhésion de l’ensemble du groupe des femmes ciblées avec des chiffres de participation insuffisants. Dans les Alpes-Maritimes ce taux est de 42,2% en 2012, inférieur au taux national de 52.7%. Une plus grande implication du médecin généraliste (MG) pourraît peut-être améliorer cette participation. L’objectif principal a été d’examiner les pratiques de dépistage des MG, de mettre en avant leurs motivations, leurs réticences, leur opinion sur ce dépistage et son déroulement afin de pouvoir proposer dans un second temps des pistes d’amélioration. Nous avons utilisé une méthode qualitative par entretiens semi-dirigés auprès d’un échantillon de MG des Alpes-Maritimes le plus varié possible. Ces entretiens ont été réalisés à partir d’un guide constitué en fonction des problématiques de recherche jusqu’à saturation des idées recueillies. Une analyse thématique a été réalisée pour présenter les résultats. Cette étude réalisée auprès de 14 MG, souligne l’importance du manque d’information actualisée sur le DO. Certains médecins ne se sentent pas impliqués dans la réalisation du dépistage. Les principales réticences sont des doutes sur son efficacité, un manque de temps, une communication entre Aprémas et les MG compliquée, la difficulté à convaincre les patientes, et un rôle non central dans sa réalisation. Ce rôle souhaité de coordinateur des MG (ce qui est le cas dans le dépistage du CCR), pourrait passer par la prescription initiale du DO. La pratique du dépistage individuel (DI) reste ainsi largement utilisée au détriment du DO. Cette enquête a permis d’identifier plusieurs pistes d’amélioration comme la création de consultations mieux valorisées dédiées au dépistage, un passage plus régulier des déléguées de Santé Publique axé sur le DO du cancer du sein, la sensibilisation des femmes les moins touchées par le dépistage (celles âgées de 70 ans et plus et celles provenant de milieux socio économiques bas), par une information médiatique promue par APREMAS, le développement d’action d’information au niveau départemental, l’implication de façon active des MG installés depuis longtemps, pour lesquels le DO n’existait pas pendant leur cursus universitaire, par exemple en les invitant à participer à des réunions grand public ou à la campagne “Octobre Rose”. Enfin d’autres idées ont émergé plus difficiles à mettre en place comme le déremboursement du DI.En conclusion, la majorité des MG souhaitait que les campagnes de dépistage perdurent mais ils voulaient être rassurés quant à son efficacité et avoir les moyens de communiquer auprès de leurs patientes.
