CONTEXTE Depuis octobre 2017, des pictogrammes grossesse « CE MEDICAMENT + GROSSESSE = DANGER » ou « CE MEDICAMENT + GROSSESSE = INTERDIT » sont apposés sur le conditionnement extérieur des médicaments pouvant avoir un effet tératogène ou foetotoxique. Ces pictogrammes ont pour but de sensibiliser les patientes sur le risque de consommer certains médicaments pendant la grossesse. OBJECTIF L'objectif de cette étude était d'évaluer l'impact chez les femmes en âge de procréer des pictogrammes grossesse apposés sur le conditionnement extérieur des médicaments sur la notion du risque de certains médicaments. METHODES Réalisation d'une étude transversale descriptive, d'avril à juillet 2019, par le biais d'un questionnaire en ligne, ciblant les femmes de 18 à 45 ans, enceintes ou non, se rendant chez un médecin généraliste, une sage-femme ou en pharmacie d'officine. Le critère de jugement principal était d'évaluer la proportion de patientes ayant connaissance du pictogramme grossesse apposé sur les boîtes de médicaments. Le critère de jugement secondaire était de mesurer le pourcentage de patientes ayant répondu « Tout à fait d'accord » à l'affirmation «Ces pictogrammes dissuadent de consommer des médicaments si l'on est enceinte ». RÉSULTATS 186 questionnaires ont été exploités, 91% (n=170) des femmes avaient connaissance de ces pictogrammes, 67% (n=125) des femmes étaient « Tout à fait d'accord » pour dire que ces pictogrammes dissuadaient de consommer le médicament. En situation de grossesse, 84% (n=154) des femmes déclaraient qu'elles prendraient avis auprès d'un professionnel de santé si un médicament avec un pictogramme leur était prescrit. CONCLUSION Le pictogramme est connu par les femmes en âge de procréer. Sa place sur la boîte est stratégique, il remplit en majeure partie son rôle d'alerte. Cependant il ne doit pas être un mode d'information exclusif et la place des professionnels de santé dans la prévention face aux médicaments tératogènes ou foetotoxiques reste entière et doit être renforcée.

Child and adolescent psychiatry hosts a range of diverse epistemological positions regarding the origin of psychical suffering, from fully endogenous (e.g. genetic) to mostly exogenous (e.g. family trauma, etc.). The complexity of clinical situations generally precludes such epistemologies to require exclusive therapeutic strategies: psychodynamic psychotherapy can be fruitful in the context of monogenic genetic illnesses (at the family or individual level), while pharmacology can be a necessary tool in a variety of difficult relational contexts or personality issues (e.g. in adolescence). Thus nowadays, the most promising therapeutic perspectives in child and adolescent psychiatry attempt to do justice to the polyfactorial complexity of mental suffering (notably by refining their psychopathologies), by drawing on e.g. biopsychosocial or epigenetic models – even more so as prevention policies ask for longitudinal studies to help with the early detection of potential future troubles.
