Childhood obesity has become a significant national health issue due to rising incidences and associated chronic medical conditions. School nurses across the United States of America are in prime positions to address childhood obesity as they have access to large numbers of children and adolescents in school settings. A review of current literature demonstrated that school nurses were overall knowledgeable concerning childhood obesity. But they encountered numerous barriers in enacting weight-related assessments and treatment programs. The purpose of this research study was to determine the knowledge, opinions, perceptions, and practice measures of South Carolina school nurses regarding childhood obesity. The School Nurses' Perceptions of Childhood Obesity Tool questionnaire was sent electronically to school nurses in South Carolina. Results from the questionnaire indicated that school nurses in South Carolina understood the causes and negative health effects of childhood obesity. Despite this awareness, the school nurses were not actively conducting weight status screenings or developing weight management programs for students.

Childhood obesity is a multifaceted, complex problem that is associated with many other chronic health conditions. School nurses are well positioned to assess, intervene, and evaluate efforts to positively impact this population, however school nurse practice for obesity varies greatly. The purpose of this dissertation was to examine concepts related to the role of school nurse providing care to school age children with obesity using both quantitative and qualitative methods. Study 1 examined the self-reported physical and psychosocial quality of life of school age children with obesity and included descriptive comments from the school nurses providing their case management. The study found that the students with obesity had lower self-reported quality of life when compared to a healthy weight sample and comments from the school nurses reported the complex physical and psychosocial dynamics for students with obesity in the school setting. These findings supported that school-based interventions need to consider both physical psychological issues experienced by students to lead to successful outcomes. Study 2 investigated the experiences of school nurses providing care to students with overweight or obesity including perceived barriers and ways that school nurses successfully implement interventions with these students. The findings of this study suggest that limited time, heavy workload, limited resources, and supervisor pressure may prevent obesity interventions thus leading school nurses to use a "seize the moment" approach where they use any contact time with students to provide interventions and suggest that activities requiring small amounts of time may be best suited for successful implementation by school nurses. Nurses in this study described experiences associated with moral distress, a concept that has not been previously studied in school nurses. Study 3 examined the level of moral distress that exists in school nurses and its relationship to common moral dilemmas and school nurse characteristics. School nurses were found to experience moral distress and it was strongly related to many of the moral dilemmas experienced by school nurses. This study provides an awareness of this issue for school nurses and includes policy implications for school nursing practice.

Abstract: Obesity is typically defined in terms of a person's Body Mass Index (BMI), a measure of excess body fat, and is characterized by a BMI greater than or equal to the 95th percentile or having a BMI of 30.0 or higher. Existing research indicates that over the past 3 decades, the percentage of overweight school-age children has nearly quadrupled. From 1999 to 2000, more than 15% of youth ages 6-19 were obese, and more than 10% of children ages 2-5 were obese. Studies have shown that obesity is related to a variety of medical and social-emotional problems which may continue through adulthood. In light of existing research, it appears that childhood obesity is a pressing issue that ought to be addressed by schools. Schools offer food services, physical education, and after-school care and have accepted some responsibility for nutrition and physical education. Furthermore, in contrast to clinical programs, school programs can be delivered at little or no cost to families and can reach low income children who otherwise may not receive treatment. The school environment is also optimal because large numbers of children can be targeted in a cost-effective manner, and instruction is a natural part of the school day. Within the school setting, school psychologists' training and guidelines for practice make them excellent candidates for taking a leading role in preventing and combating obesity. The present study sought to identify the attitudes and perceptions of school psychologists in Ohio regarding their role in dealing with childhood obesity. A 19-question survey, along with a cover letter, was mailed to 147 randomly selected, Nationally Certified School Psychologists (NCSP) in Ohio. Some survey items were phrased as questions that are answered according to a Likert-type scale ranging from "not at all" to "a great deal." Other questions provided a list of responses and required that participants circle all applicable responses. Following the survey questions, participants were given space to provide additional comments. Following a first and second mailing, 107 completed surveys were received, resulting in a return rate of 72.8%. Analysis of the data involved comparing the frequencies and percentages of the responses provided by participants. Over half (58.9%) of participants reported that they have not been involved in obesity-related work. When asked to indicate which individual(s) should be responsible for dealing with childhood obesity, over half (59.8%) indicated that school psychologists should be responsible. In addition, when asked to indicate the extent to which obesity issues should be handled by school psychologists, the majority of participants (68.2%) selected the response "to some extent." Additional comments provided by participants were also reviewed and summarized. Based on the data gathered in the present study, it was concluded that the majority of school psychologists believe that they should be involved in dealing with childhood obesity; yet the majority are not involved in obesity-related work. Future studies may further investigate the barriers to school psychologists' involvement and identify means of achieving more involvement among school psychologists. The effectiveness of obesity-related work by school psychologists should also be determined.

Childhood obesity is a significant health problem. This mixed methods study used an online survey to investigate senior baccalaureate nursing students’ knowledge about childhood obesity, attitudes toward, and their perceived self-efficacy in addressing childhood obesity as a health problem. Attribution value theory and social cognitive theory were used. A sample of senior BSN students (N=102) completed the survey. This study investigated whether child weight, gender, and physical activity level or parent weight influenced health promotion and attitude towards the child and parent. Student self-efficacy and educational preparation to address weight as a health problem was also investigated. Randomized multiple segment vignettes featuring a five and a 15-year old were used with open-ended questions and semantic differentials. An overweight child was more likely to elicit nutrition as a health promotion topic, X(1)=4.386, p=.037. The weight of the child was significantly related to the number of weight-related topics introduced, X(7)=29.35, p=.000. The gender of the adolescent was significantly related to the number of weight related topics, X(7)=91.201, p=.008. BSN students have more negative than positive attitudes towards overweight children and overweight mothers. Low child and adolescent activity was also associated with more negative attitudes. Students reported being confident in their abilities to work with children and families with childhood obesity. Students described varying foci of their education regarding childhood obesity including nutrition and health effects of obesity. These findings have implications for nursing education. However, more research is needed.
