Le fait que les patients présentant une démence aient des troubles du comportement de la communication, des conduites sociales et une indifférence affective nous a incité à rechercher des troubles de la perception des émotions pouvant contribuer à expliquer les modifications comportementales. Notre travail a montré qu’il existe un trouble de la perception des émotions sur les visages chez les patients présentant une démence fronto-temporale. Les patients présentant une maladie d’Alzheimer ne sont pas différents des témoins à un stade précoce mais les troubles se majorent avec l’aggravation de la maladie. Nous avons par ailleurs montré que la perception des émotions sur les visages s’altérait avec l’âge, était meilleure chez les femmes et pour les sujets de niveau culturel élevé. Afin d’apporter des arguments concernant les bases neuropsychologiques de la perception des émotions, nous avons étudié des populations de patients présentant des asymétries morphologiques ou fonctionnelles. Les patients présentant une démence fronto-temporale ou une épilepsie frontale ou temporale n’ont pas permis de mettre en évidence une organisation unique et systématisée de la perception des émotions en concordance avec les études précédentes et quelle que soit la méthode utilisée ; nous en avons conclu que les structures impliquées comprennent les lobes frontaux et temporaux et qu’il existe probablement une asymétrie fonctionnelle sous tendant les différentes émotions. D’autres études sur les lésions très focales avec des paradigmes expérimentaux simplifiés dans une seule modalité perceptive pourraient permettre d’étayer cette hypothèse.

To a certain extent the dementias have been forgotten diseases until just recen tly when they were brought to the attention of the general public and health authorities as a result of the increasing number of cases in the aging popula tion, especially among famous people, and because of the efforts of private foundations.
