Contexte: Le langage de l'enfant fait partie intégrante de son développement global. Les troubles du langage doivent être repérés précocement pour assurer une prise en charge adaptée. Le médecin généraliste, qui suit l'enfant dès le plus jeune âge, est un acteur primordial dans l'évaluation du langage de l'enfant. A l'âge de 2 ans, l'enfant est en période dite d'explosion lexicale et acquiert la combinaison des mots. C'est également l'âge de la consultation obligatoire de santé des 24 mois mais il n'y a pas, à ce jour, d'outil commun pour le repérage des troubles du langage oral de l'enfant à cet âge. L'Inventaire Français du Développement Communicatif (IFDC) version courte de 24 mois est un questionnaire parental validé qui permet d'évaluer le vocabulaire expressif des enfants à cet âge. Cet outil est disponible sur le site internet du ministère de la santé. Objectif: Dans cette étude, nous proposons aux médecins généralistes, maîtres de stages des universités du Nord-Pas-de-Calais, de nous informer sur leurs pratiques actuelles et de donner leur avis sur l'utilisation de l'IFDC -version courte- 24 mois. Méthode: Enquête déclarative transversale auprès de 221 médecins généralistes, maîtres de stage de l'université de médecine de Lille et de la faculté libre de médecine de Lille, par un questionnaire envoyé et réceptionné par mail, de manière anonyme, après accord de la CNIL. Résultats: 105 médecins sur 221 ont répondu au questionnaire de thèse soit 48%. L'activité pédiatrique de moins de 2 ans représente 1 à 15% de l'activité médicale pour 71% IC [67; 75] des médecins. 77% IC [69-85] d'entre eux reconnaissent l'importance de l'évaluation du langage à cet âge, notamment au cours de la consultation des 24 mois qui est réalisée systématiquement pour 36% des médecins interrogés et pour plus d'un enfant sur 2 pour 45% d'entre eux. Cependant, le temps dévolu à l'évaluation du langage est restreint au cours de la consultation. Les médecins interrogés connaissent très peu d'outils d'évaluation du langage à 2 ans : seul 7 % IC [2; 12] d'entre eux ont cité un outil connu. 85 % IC [78; 92] des médecins interrogés expriment un avis favorable à l'utilisation de l'IFDC - version courte- 24 mois qui leur était inconnu pour 98%. Le frein essentiel à son utilisation est le manque de temps cité par 72% IC [63; 81] des médecins interrogés. Du coté des parents, les médecins estiment que le premier frein est la compréhension du questionnaire par la famille. Il ressort de cette étude peu d'obstacles pratiques à l'utilisation de l'IFDC -version courte- 24 mois pour le médecin généraliste. Cependant, 56% IC [47;65] des médecins souhaitent utiliser ce test en cas de doute diagnostique et 15% IC [8;22] de manière systématique au cours de la consultation des 24 mois.
