L’importance de la production locale dans l’amélioration de la santé publique et dans le développement industriel a été notée depuis les années 1970. Le transfert de technologie a été perçu comme un moyen efficace d’encourager cette production locale et de diminuer les disparités Nord-Sud. Il permet également d'améliorer l’accès aux médicaments des populations des pays en développement accueillant la technologie transférée. Outre le cédant et le cessionnaire, le transfert de technologie met en jeu différents acteurs. Ainsi, les organisations multilatérales (OMS, ONUDI, IFC, CNUCED,...), les gouvernements et les ONG ont tous un rôle à jouer afin d’assurer la réussite d’un transfert. Le transfert de technologie est régi par un certain nombre d’accords, comme les accords de confidentialité, les accords de transfert de matériel, les accords de cession, les contrats de licence et les alliances stratégiques. A ce jour, si rien n'oblige les entreprises pharmaceutiques à entreprendre des transferts de technologie, diverses initiatives ont été prises. Par exemple, au Brésil, certaines initiatives datent de 1985 et sont toujours d’actualité. D’autres pays, comme la Jordanie et l’Indonésie, sont devenus en quelques années de véritables plateformes produisant et fournissant des médicaments aux populations des pays de leurs régions.
