Objectif : l'objectif de cette étude était de déterminer l'effet d'un protocole d'optimisation hémodynamique peropératoire sur la réduction du taux global des complications en chirurgie de reconstruction maxillo-faciale par lambeaux libres. Méthode : soixante-six patients ont été inclus dans cette étude observationnelle avant-après. La prise en charge hémodynamique dans le groupe avant était traditionnelle. La prise en charge hémodynamique dans le groupe après était basée sur un protocole associant fluid challenge guidé sur l'analyse du contour de l'onde de pouls et usage raisonné de vasopresseurs afin de maintenir une PAM > 70 mmHg. Le critère de jugement principal était un score composite global des complications péri-opératoires.
